8ffceea8e3 swarm-ci-runner: load the registration token in a register unit of its own
systemd hands a unit's credentials to every process of that unit, and a
host job is a process of the runner daemon, so the `LoadCredential=` on
gitea-runner-swarm still let any workflow read the registration token
from `$CREDENTIALS_DIRECTORY`.

swarm-ci-runner-register.service, a oneshot under the daemon's dynamic
user and state dir, now holds the only `LoadCredential=` and runs the
precond and `act_runner register` (only when `.runner` is missing). The
daemon unit has no credential, no `EnvironmentFile` and no `TOKEN`; its
pre-start step starts the register unit, so a missing token fails the
daemon's start and `Restart=on-failure` retries it.

The precond compares `.runner`'s labels in the form the daemon rewrites
them to (`nix` -> `nix:host`); compared verbatim, a scheme-less label
re-registered the runner on every start.

tmpfiles removes the unused `registration-token.env` and upstream's
`.token-hash`/`.labels` markers.

0302b29ca4 swarm-ci-runner: give the registration token only to the register step
The runner got its registration token through an `EnvironmentFile`
(`TOKEN=…`), so the daemon carried it for its whole life, and for a
`host` label act_runner copies its environment into every job step:
any workflow could read a token that registers runners on the forge.

The host unit now writes the bare token, and the runner unit loads it
with `LoadCredential=`. The upstream pre-start step, which registered
from `$TOKEN`, is replaced by one register step that reads the token
from `$CREDENTIALS_DIRECTORY` and runs `act_runner register` only when
`.runner` is missing, with the instance URL, name, labels and config
file upstream used. The precond now also removes `.runner` when its
labels differ from the configured ones, which upstream's step covered.
A changed token alone no longer re-registers.

Host jobs can still read `.runner` itself (#4979).

84347f122c forgejo: actionsFromSelf defaults to whether this host runs the swarm-controller
The controller seeds the actions/checkout mirror from its own host's
mirror list, so a forge host without it got DEFAULT_ACTIONS_URL = self
and no mirror. The default now follows
deploy.swarm-controller.enable; the module-eval check pins forge-only
(false, no warning) and forge+controller (true, mirror seeded).

e2da5509ae swarm-ci-runner: run the forge CI runner as a swarm service
The CI runner was a hive deploy module: hive-ci.nix ran it beside the
forge, and hive-c0re registered it through the forge's admin API with
the `core` token, writing the registration token through hive-priv. It
could only run on the forge's host, and it kept an admin-scoped forge
token on a hive.

Now the runner is a swarm service:

- swarm-ci-runner.nix runs it in a `swarm-ci-runner` container on the
host that sets deploy.ci-runner.enable, reaching the forge by name.
It needs neither the forge nor a hive there.
- swarm-controller reads the forge's runner registration token with its
own admin token and writes it to swarm/services/ci-runner/
registration-token whenever it differs from what it last wrote. Its
grant on that path is create/update only.
- The runner's own leaf (CN swarm-ci-runner, role and policy of the same
name) reads that one path. swarm-bao-ci-runner.service fetches it into
the runner's EnvironmentFile and restarts the runner on a change.
- A pre-start step removes .runner when the daemon's journal since its
last start holds Forgejo's "unregistered runner" refusal (the daemon
exits 1 when its startup Declare gets it), or when .runner names
another forge address; upstream's pre-start step then registers again.

Removed from the hive side: hive-ci.nix and deploy.forgejo.ci.*,
forge/ci_runner.rs, PrivRequest::RegisterCiRunner and its handler and
ReadWritePaths entry, InfraContainer::Ci with hivectl's --ci and the
dashboard's hive-ci journal entry, HYPERHIVE_FORGE_CI_ENABLED, and
read:admin,write:admin from CORE_TOKEN_SCOPES: nothing else in hive-c0re
calls /api/v1/admin/*. The old ci options, under both swarm.forge.ci and
deploy.forgejo.ci, now fail evaluation naming deploy.ci-runner.

The forge's half of the old ci.enable, the actions/checkout mirror and
DEFAULT_ACTIONS_URL = self, is deploy.forgejo.actionsFromSelf.

ac592a5d23 forge: always behind the gateway; drop behindGateway
The forge always sits behind the gateway, so `deploy.forgejo.behindGateway`
(and its `swarm.forge.behindGateway` rename alias) is removed and its
true-branch behaviour is now unconditional within `deploy.forgejo.enable`:
https ROOT_URL on the gateway's httpsPort, the forge vhost and local DNS
name, the swarm-ui quick link, the published metrics scrape target, forgejo
metrics, the authelia `/metrics` rule, and `publicUrl` defaulting to
`https://<forge.domain>`.

Removed with it: the direct-port `http://<domain>:<httpPort>/` ROOT_URL
branch, the hive-ci assertion that the option is true, the core-toggle
cases that only exercised the false branch (the services-leaf case reads
`bare`, which never enabled the forge either). `hivectl open forge` now
points at `swarm.forge.publicUrl`, which can still be set to null.

54556e4661 docs(networking): fix round — argus review + audit E1/E2
observability.md: "Why two tiers" claimed the harness currently writes an
upstream token into the agent's own claude settings; that path was removed
with the direct-export mode it served (nix/agent-modules/otel.nix:56-63).
Reworded as the hypothetical the paragraph is actually making.

gateway.md: restored the agent-trust pointer to
/run/hive-ca/trust-bundle.pem in "Cert prompts" (hive-ca-trust.nix:41),
dropped by the earlier rewrite. Corrected the SPA-fallback section: only
the chat.<swarm> vhost uses the Accept-header map
(hive-matrix.nix:693-696); per-agent split mode uses file-existence
try_files (gateway_nginx.rs:93-134), not the same mechanism.

067f4e5699 docs(networking): facts + structure pass on gateway, network, jobq, observability, matrix
gateway.md: split the opener into what/audience/enable; vhost map in two
tables (swarm-service vhosts declared by their own modules, then the hive
vhost) matching vhosts.nix and the service modules; gateway.enable exists
and is set with mkDefault by the modules that need it; Basic auth scope,
dashboard /health/ prefix, error-page rendering, matrix body limit and
forge link source corrected; nginx internals grouped under one Internals
section with their headings unchanged.

network.md: gateway and dnsmasq run on the host, not in a container;
network.enable is set by the modules that need it; shared-netns firewall
rule covers every swarm service container; hive-priv writes the nspawn
conf; domain sentence rewritten; removed options moved into <details>.

jobq.md: swarm-controller runs its own graph; swarm UI /jobs and BU1LDS
show different graphs drawn by the same component.

observability.md: swarm tier first; history narration cut; network access
deduplicated into a link to network.md; options link made absolute.

matrix.md: swarm.matrix vs deploy.matrix namespaces; tuning, firewall and
SSO options under deploy.matrix; .well-known is served on the hive domain;
roadmap sentence deleted; stale hive-c0re provisioning claims fixed;
serverName upgrade note moved into <details>.

c2bdf30e05 hive-agent: export ACP-reported cost and context fill over OTLP
An ACP agent's `usage_update` carries `cost.{amount,currency}`, the
session's running total (opencode sums every assistant message in the
session). hive-runtime now reads it and turns the running total into
what each report added: a new session counts from zero, a session loaded
into a freshly started agent only baselines on its first report, and a
falling total adds nothing. The spend is held on the runtime until
`Runtime::take_reported_cost` drains it; claude's runtime reports none,
since the claude binary already exports `claude_code.cost.usage`.

hive-agent's existing turn-metrics meter records three new instruments:

- `hyperhive.agent.cost.usage` (counter, `model` + `currency`), ACP only;
- `hyperhive.agent.context.used` / `.size` (gauges, no attributes), for
every backend: the two numbers the web UI's ctx% divides.

The `hyperhive · agents` dashboard gets ACP cost panels on its cost tab
and a context-fill panel on its health tab.

addresses argus's review on #4842 (c88740): fetch's success path was
silent when the response parsed but windows() found nothing to record.
should_warn_empty tracks the same once-per-streak shape as the
existing token-skip logging, reset the moment a response has windows
again, and never logs the body.

Also notes in the observability doc that a past resets_at means the
paired percent gauge is stale.

8a735ddcb3 hive-agent: publish Claude subscription usage (5h/7d %) as metrics
A detached task polls GET https://api.anthropic.com/api/oauth/usage every
5 minutes with the OAuth access token from ~/.claude/.credentials.json and
records, per window the response names (five_hour, seven_day,
seven_day_sonnet, ...):

- hyperhive.agent.claude_usage.percent (%, 0-100)
- hyperhive.agent.claude_usage.resets_at (s, unix seconds)

both labelled window=<name>. Endpoint, the anthropic-beta:
oauth-2025-04-20 header and the {utilization, resets_at} window shape are
taken from the claude-code 2.1.283 bundle's own /usage fetch.

The token is only read, never refreshed: claude owns refresh-token
rotation and a second refresher can log the agent out. An expired token
is skipped until claude's next turn refreshes it. API-key agents
(HIVE_USE_API_KEY, the ACP default) and agents with no credentials file
skip quietly, and the task does nothing when OTEL is not configured.
Request failures and non-2xx statuses warn with the status or transport
error only, never the body or the token.

5785c0024c Make agent creation swarm-only and refuse a name placed on another hive
swarm-controller's POST /api/agents now refuses (409) a name the swarm
has already placed on a different hive: a non-Destroyed declaration in
that hive's wanted state, or a SetAgentWanted node still queued for it.
The same name on the same hive is that agent being re-created and goes
through. A wanted state that cannot be read refuses (503/500) instead of
reading as "placed nowhere". Creations are serialised from that read to
the graph insert so two concurrent creations of one name cannot both
pass.

Hive-level creation is removed: hivectl `agent create` / `request-create`,
HostRequest::Spawn / RequestSpawn, the dashboard POST /api/request-spawn
route, and ApprovalKind::Spawn with its approve/resolve arms and the
approval-carrying `templates::spawn`. The swarm path (deploy request or
wanted-state sweep -> queue_first_deploy -> templates::first_deploy) used
none of them. Old `spawn` approval rows are skipped by collect_lenient,
as `init_config` rows were in a3b672d1.

policy.rs's comment on agent_object_name stated swarm-wide name
uniqueness as a fact; it now says where it is enforced and what that
check cannot see.

313d582c01 job_queue: drop insert_unless_live, accept extra queue passes
mara chose to accept extra queued sweep passes over adding a new
hive-jobq primitive (or a hive-c0re one-off) for "don't queue another
of this kind". Every sweep caller now plain-inserts its node; the
capacity-1 Dep::Resource per sweep kind (MatrixSweep/KnowledgeTree)
still keeps two passes of the same kind from running concurrently, it
just no longer collapses a tick that lands while one is live or queued
into the existing one.

1d4c77d2c8 hive-c0re: serialise the matrix and knowledge sweeps through the job queue
The matrix sweep and the /knowledge pull each had concurrent callers
(#4723 item 4). Two overlapping knowledge pulls fail on .git/index.lock
and the remote-tracking ref lock: 30 of 30 concurrent replays of the
reset/clean/pull sequence in a scratch repo errored, 0 of 10 sequential
ones did. Two overlapping matrix sweeps on a hive with no persisted
Space / chat-room id both miss the by-name lookup and both createRoom
(from reading the code, not reproduced against a homeserver). On every
boot the MatrixSweep DAG node and the main.rs loop's immediate first
call ran at once.

Every sweep now runs as a job node, and each sweep's node holds its own
capacity-1 queue resource (Resource::MatrixSweep,
Resource::KnowledgeTree), the MetaWindow pattern: the scheduler never
starts a second pass of one sweep while the first holds the resource,
and different sweeps still run side by side.

- templates::matrix_sweep / templates::knowledge_pull build the node
with its resource; boot, the periodic loops and the swarm event all
  use them.
- JobQueue::insert_unless_live folds a submission into a live node of
the same kind instead of queueing another. Periodic ticks fold into a
queued or running pass. The swarm knowledge event folds into a queued
pull only, and queues one behind a running pull, which may have
  fetched before the push.
- The main.rs matrix loop no longer sweeps immediately at startup; the
boot MatrixSweep node is the startup pass, as KnowledgePull already
  was for knowledge.
- The executors bound each pass (10 min matrix, 5 min knowledge), since
a hung pass would otherwise hold its resource against every later one,
and own the sweep-health banners, so every pass reports to them.

Replaces the SweepLock version of this branch, per review.

2252c55df8 hive-priv: create agent socket dirs on start; drop hyperhive-agents.conf
/etc/tmpfiles.d/hyperhive-agents.conf was a boot-time backstop (#2290)
that pre-created every agent's bind sources. The start preamble already
creates them for every c0re-driven start, and on this host only hive-c0re
starts agent containers. The file was also the reason the socket dir's
owner had to be declared there, which is how it spent its life at
`0777 root root` whenever the uid could not be resolved (#4742).

- hive-priv gains `EnsureAgentSocketDir { name }`, called from
`set_nspawn_flags` in every start path. It creates
`/run/hive-agent/<name>` `0751 root:root` with mkdirat relative to an
O_DIRECTORY|O_NOFOLLOW fd for the parent. An existing entry has to be a
directory (fstatat AT_SYMLINK_NOFOLLOW); anything else is refused, and a
directory is left alone. hive-c0re's own create_dir_all went: its /run
  is read-only under ProtectSystem=strict.
- The container's `hive-agent-user-migrate` activation chowns that dir to
the agent user and sets 0751, the same way it already handles state/ and
  harness/. It refuses a symlink or non-directory there, since `test -d`
and chmod follow links. No host-side passwd parse, and no window where
  the dir is world-writable.
- `/run/hyperhive/agents/<name>` stays created by hive-c0re itself
  (`ensure_agent_runtime_dir`). It holds the `mcp.sock` that hive-c0re
binds as hive-core, so it must not become root- or agent-owned.
- The `/run/hive-agent` parent is declared in hive-priv.nix, `0755
  root:root`, instead of hive-gateway's hive-core rule. hive-priv is its
only writer now, and hive-priv's ReadWritePaths needs it to exist.
- The manager start in `ensure_root_agent` now goes through
  `converge_start_preamble` + `start_with_fallback`. It was a bare start,
so after a reboot the manager's bind sources existed only because of the
tmpfiles file, and its limits drop-in did not exist at all.
- Removed: `sync_tmpfiles`, `agent_uid_gid` / `parse_passwd_uid_gid`,
  `priv_client::sync_agent_tmpfiles`, `AgentTmpfilesEntry`, the tmpfiles
body builder and their tests, plus the three call sites.
- Legacy: hive-priv unlinks the file at every start, ignoring ENOENT.
`SyncAgentTmpfiles` stays one release as a payload-ignoring variant that
does the same unlink and returns Ok, for an older hive-c0re.

Salvaged from #4752: the boundary.md correction that nginx only dials,
because ProtectSystem=strict makes its /run read-only.

Behaviour change: a manual `nixos-container start h-<name>` right after a
reboot, before hive-c0re has started that agent, now fails on a missing
bind source instead of starting.

20419ccd41 swarm-controller: own the swarm-wide forge objects; hive-c0re stops creating them
The orgs agent-configs/internal/agents (plus mirror owners), the
operators team in agents and agent-configs, the pull-mirrors,
internal/docs, internal/knowledge (public, README-seeded) and the
agent-configs org avatar are one set per forge. hive-c0re ensured them in
its boot sweep, as the core admin, and only on the hive co-located with
the forge container.

swarm-controller now reconciles them at start and every 5 minutes
(forge/objects.rs: observe -> pure plan -> apply). A failed object logs
a warn line plus a pass summary and is retried next tick. create_repo
ensures the agent-configs org and its operators team first, so a config
repo's merge gate never depends on the periodic pass having run.

hive-c0re drops ensure_org, SEEDED_ORGS, ensure_mirrors/ensure_mirror_repo,
ensure_operators_team, ensure_shared_docs_repo, ensure_knowledge_repo/
set_repo_public, seed_readme, ensure_config_org_avatar and the one-shot
knowledge::remove_webhook cleanup, with their now-unused helpers.

nix: the mirror list moves from the hive-c0re unit

declared on a host that runs no controller. c0re.orgAvatarPng is renamed

978164dc53 nix: run the forge on one host per swarm (deploy.forgejo.enable)
Every hive with hyperhive enabled ran its own hive-forge container, and

deploy.forgejo.enable defaults to false and allSwarmServices sets it with
mkDefault, like authelia and bao; singleHostSwarm gets it through that.
The forge's OIDC client moves to a glue module gated on authelia, so a
split authelia/forge swarm still registers it. CI now requires the forge
on the same host, and the controller's forgeTokenFile defaults to null

8e9ca3ee6c grafana: tell the logs datasource that severity_text is the level
Grafana's log-level buttons filter on a field called `level`; no row in
the store has one. The store's name for it is `severity_text`, chosen by
VictoriaLogs' OTLP ingester rather than by us — v1.52.0's
`app/vlinsert/opentelemetry/pb.go` writes it unconditionally and the
ingest parameters have no `_level_field` to rename it with. So the
mapping is made on the reader: the VictoriaLogs datasource, which was
provisioned with no `jsonData` at all, now carries a `logLevelRules`
entry per severity the journald parser can emit.

`logLevelRules` is the datasource plugin's only level-related jsonData
key — there is no field-name setting and no OpenTelemetry preset to
switch on. It is read off `instanceSettings.jsonData` in the plugin's
`datasource.ts` and typed in its `configuration/LogLevelRules/types.ts`,
both recovered from the sourcemap shipped in the pinned artifact
(`grafanaPlugins.victoriametrics-logs-datasource` 0.26.3). Each enabled
rule appends an `OR severity_text:="INFO"`-shaped term to the query a
level button emits, next to the `level:…` term that matches nothing.

A wrong rule here fails silently: Grafana provisions unknown jsonData
without complaint and the buttons go on returning zero rows. The three
ways to get one wrong — a non-literal `enabled`, a non-canonical `level`
spelling, a value that is not the stored text — are recorded at the
binding, and a module-eval arm pins them along with the real failure
mode, a severity added to `nix/journald-severity.nix` and not here.

`Unspecified` is left unmapped on purpose: it is the store's own
rendering of an absent severity and the thing the logs dashboard's "no
severity" panel counts.

substring-match artifact — see hyperhive#4548's per-PR bodies for the
per-hit reasoning).

Wraps each one in a scoped <!-- vale write-good.Passive = NO/YES -->
pair (the supported mechanism — TokenIgnores has a known offset-drift
bug) rather than a blanket per-file or per-rule silence, so a *new*
passive-voice hit anywhere in these files still fails once the rule
gates CI (next commit). Table/list false positives (docs/swarm/credentials.md's
renewal-table cells) wrap the whole block, not each cell.

f047e509a7 docs: clear write-good.Passive hits in docs/scheduler/coordinator.md
5 genuine passive-voice rewrites (actor named — nixos-container
create; the passive construction replaced with an active
intransitive verb or 'nothing can X' phrasing matching the pattern
already established on credentials.md); 4 false positives left
untouched — a type-description idiom ('is typed'), a predicate-
adjective in a parallel triple ('is satisfied' alongside two
untouched siblings), and two already-established categories
('is unaffected', 'is tracked' as a follow-up).

f45cecf08c ops: address argus review findings on PR #4433
Add attribution-trailer lint row to the ci.md checks table (modeled
on the vale-errors row: not yet a required check) so the doc doesn't
assert stale completeness once attribution-trailers merges. Delete
the dead CI_COMMIT_REF_NAME elif in check-attribution-trailers.sh —
byte-identical to the else arm, never exercised, and its comment
overclaimed GitLab/Forgejo support nothing here can test.

78a53cc9ac docs/observability: hive→swarm ingest auth shipped, stop calling it planned
The security section said hive→swarm ingest auth was "planned" and that the
`hive` label "becomes" unforgeable. Sixty lines later, `### Authenticated
ingest` in the same file describes it as shipped and mandatory — "No
unauthenticated mode exists" — and the code agrees: swarm-otel.nix:157 derives
one receiver port per entry in `swarm.hives`, :1387 stamps `key = "hive"` from
the receiver that accepted the sample, :751 asserts `swarm.hives != {}`, and
otel.nix:659 makes a hive without an identity a build error.

Not drift. Ordered by position in main rather than by author date, the "planned"
wording is the NEWEST of the three commits: `9dc60061` documented authenticated
ingest and `9bd2b9e9` dropped the unauthenticated mode (both 2026-08-19), while
`5fcd2a93` — authored 00:59 that morning, merged on 08-30 — added the sentence
eleven days later into a tree where the feature already existed. A long-lived
branch's prose is a snapshot of the tree it was written against, and nothing
re-reads it at merge time.

Matters because §"what the agent→collector hop is and isn't" is the page a
reader goes to for "can a hive forge a label". It answered "auth is planned",
which reads as "`hive=` is forgeable today". The truth is the opposite and
stronger.

0d31560e38 docs/jobq: the queue view hides done and skipped steps by default
jobq.md's "Watching it happen" section ended by promising that a step which
isn't needed "shows as `·` rather than dropping out of the tree entirely, so
the same kind of operation keeps a recognizable shape run to run".

In that view it does drop out. `JobqGraph.tsx:94` sets
`DEFAULT_HIDDEN_STATES = {Done, Skipped}` under the comment 'Product call:
"default selection filters out skipped and done"', `:299` seeds the selection as
everything except those, and `:271` puts the selection in the query string — so
the hidden states are never fetched, not merely styled out. That component is
what renders both surfaces the page names, and neither passes an override.

The claim is true of the data and false of the screen, in a section about what
is on the screen. Keeping both: the node stays in the graph, and the default
filter is named, along with the part that would otherwise surprise someone
debugging it — the selection is a request parameter.

ccde8a2445 docs: the agent telemetry hop carries logs now, not only stats
observability.md scoped the whole feature to Claude Code statistics, so a
reader asking what leaves an agent container would have concluded
counters and nothing else.

That page also documents the trust properties of the agent->collector
hop, and those are what needs restating for a richer payload: same
destination, same credential, same already-accepted ability to push
arbitrary OTLP. What changes is how much detail leaves by default.

docs/swarm/services.md needs no change. It already said each hive ships
the journals of "the harness units inside every agent container" — which
was not true until now.

55f01942a2 docs, prompts, hive-forge: stop handing readers the renamed verbs
docs/tools/forge.md already listed the nine renamed verbs as removed, then
used them ~30 more times in pasteable blocks. Sweeps every occurrence a
reader would type, including three runtime messages that told the user to
run a verb the same binary rejects.

The renamed-verb list itself keeps the old names; it is what documents them.

d2175c84b3 docs: stop citing two hivectl commands that do not exist
mcp.md called it `hivectl choom`; the verb is `hivectl agent <name>
choom`. observability.md cited `hivectl shell`, which has never existed
at any level — the sentence only needs a shell, so it names one instead of
guessing which command an operator reaches for.

Found by subtracting the generated hivectl-cli.md's command headings from
every hivectl citation in docs/. Re-running it after the fix drops the
candidate list from 16 to 14, losing exactly these two.

c13499c839 ci: split an error-only vale check out of the full prose-lint job
mara, on #4128: add a CI step (separate from the existing one) that
only flags error-level vale hits, so it can be made a required check
without also gating merges on the pre-existing warning/suggestion

Also brought docs/scheduler/ci.md's CI-checks table back in sync with
reality (it was missing the doc-pointer lint job already; now lists
both vale jobs and their required-check status).

15890f9d5b docs: fix genuine Microsoft.Contractions/Negative errors
16 of the 23 error-level hits from a full-repo vale run were real:
'is/are/does/do NOT' and bold 'not' expanded per Microsoft.Contractions,
one hyphen-as-negative-number fixed to an en dash per Microsoft.Negative
(docs/web-ui/dashboard.md's -45 deg column-header tilt).

Where the caps/bold was there for emphasis (contrasting X does this,
Y does not), kept the emphasis by bolding the contraction rather than
dropping it silently -- 'is not' -> 'isn't', 'does NOT' -> '**doesn't**',
etc.

docs/tools/swarmctl-cli.md's one genuine hit is generated -- fixed the
source doc comment in swarmctl/src/main.rs and regenerated via
'cargo run --bin swarmctl -- markdown-docs', diffed against the old
copy first to confirm only the intended line changed.

Left 7 hits unfixed, false positives from the rule matching a
substring rather than the actual pattern it names:
- 4x 'that's instead of that is': every one is the explanatory 'that
  is' idiom (~= i.e.), not a contractible subject+verb -- 'that's'
would misparse as a demonstrative-pronoun contraction and change the
  sentence's meaning. docs/integrations/forge.md:282,
  docs/tools/forge.md:260, docs/tools/hivectl.md:243,
  docs/web-ui/dashboard.md:457 (and the swarmctl-cli.md generated
twin of the same category, source left alone for the same reason).
- docs/agent-lifecycle/persistence.md:482 -- 'is not' matched inside
'is nothing' (the same has-not/has-nothing substring trap noted
before in this doc's own git history).
- docs/swarm/ca.md:182 -- 'it is' matched inside the already-correct
  'it isn't'.

Per #4128. Remaining errors on main after this: Microsoft.Avoid's 26
hits, already tracked + awaiting a house-style ruling on #4041.

fd633f674e docs: drop the comma outside the closing quote in coordinator.md
argus's review on this PR caught a real regression: the earlier
passive-voice rewrite ('answers "did the agent come back up?", the
same way...') put a comma immediately outside a closing double-quote,
which vale's Microsoft.Quotes rule flags ("punctuation should be
inside the quotes") -- a brand new error not present on main. The
original phrasing never had a quote directly followed by a comma, so
this was a real side effect of the rewrite, not a pre-existing hit.

Simplest fix: drop the comma. The sentence reads fine without it and
the closing "?" already does the job.

Verified via vale: back to 2 errors (matching main's baseline, down
from the 3 this rewrite introduced), write-good.Passive count
unchanged at 24.
