80e8361da9 agents: the send(operator) refusal also covers agents without matrix
An agent with no matrix account has no `mcp__matrix__*` tools, so a
refusal that only says "use the matrix MCP tools" points it at something
it cannot call. `OPERATOR_SEND_REFUSAL` now names matrix for an agent
that has it, and tells one without it that there is no direct channel:
the operator reads its `set_status` line, its turn output on the
dashboard, and its logs. The allow-list refusal, the `send` tool
description, the MCP server instructions and the system prompt say
"over matrix if you have it" to match, and the helper-event bullets go
back to "ask the operator" without naming a channel.

7e6a4c7727 agents: remove send(operator) and the operator inbox; alert on turn failures
Agents reach the operator over matrix now (#5115). A `send` to
`operator` is refused, by the broker and by the MCP allow-list check,
with an error that names the matrix MCP tools. The dashboard's Y3R C4LL
tab goes with it: the operator inbox, `GET /api/operator-inbox`,
`Broker::unread_for_recipient`, and the browser notifications that only
ever fired on operator-bound messages.

The harness no longer posts turn failures and plugin-install failures
to the operator inbox. Both are already logged at warn; a new Grafana
rule, `hh-agent-turn-failed`, fires per agent on the harness's
`turn failed` line.

Dashboard-to-agent messaging (op-send, the agent web UI's message box,
mark-all-read for agent inboxes) is unchanged.

Decisions per mara on #5115: q1 a, q2 a plus a grafana alert, q3 keep
dashboard-to-agent messaging and leave swarm-level code alone.

58ef299c3c hive-c0re: remove the approvals subsystem
Nothing submits an approval since scheduled prompts left hive-c0re, so the
queue only acted on rows written before that.

- hive-c0re: the approvals store and its migrations, approve/deny/
  resolve actions, the ResolveApproval tail node (and meta_update's
  approval_id), POST /api/approve and /api/deny, the approval views on
  /api/state, the approval_resolved dashboard event, the orphan GC,
  destroy/purge failing pending rows, and the approval rows in
  get_loose_ends. DestroyBookkeeping loses the purge flag, which only
  worded the approval-failure note.
- The CancelLooseEnd request goes: its only hive-c0re arm was the
  approval one. cancel_loose_end handles reminder and todo in the harness.
- Wire types: hive_sh4re::approvals (ReminderStats moves to inbox),
  LooseEnd::Approval, CancelLooseEndKind, HelperEvent::ApprovalResolved,
  ToolGroup::Approvals, HostRequest::{Pending,Approve,Deny} and
  HostResponse.approvals. HOST_SOCK_VERSION goes to 2 for the removed
  field.
- hivectl loses the approvals verbs; docs/tools/hivectl-cli.md is
  regenerated.
- The system prompt drops approval_resolved.
- The approvals table stays in broker.sqlite, unread, the same way the
  operator_questions table was left when its store was removed.
- An `approvals` entry still in tool-groups.json is skipped with a warning
  by ToolGroup::parse_list, like any unknown group.
- docs/agent-lifecycle/approvals.md is renamed to config-changes.md so the
  code's doc pointers resolve; its content follows in the docs commit.

c3a54bbdd0 hive-c0re stops owning scheduled prompts; agents lose the schedule tools
Scheduled prompts are swarm-level now (swarmctl schedule, #4936). The hive
keeps the rows it has only so the operator can copy them over.

- hive-c0re no longer fires anything: the scheduled-prompts worker is gone,
  and with it every create/edit/cancel/pause/resume/fire path (dashboard
  endpoints, agent-socket verbs, the SchedulePrompt approval kind and its
  approve handler). The store keeps open + list; GET /api/schedules stays.
- A v6 approvals migration drops schedule_prompt rows.
- Nothing submits approvals any more, so the submit path goes too:
  Approvals::submit_kind (now a test fixture), the ApprovalAdded dashboard
  event and its emitter.
- Coordinator::push_todo, whose only caller was the worker, goes, and with
  it hive_host_sock::agent_todo_socket and hive-c0re's hive-agent-sock /
  hive-sock-client deps.
- The schedules_changed dashboard event goes; nothing mutates the rows.
- Agents lose request_schedule_prompt, cancel_schedule, fire_schedule_now,
  edit_schedule and list_schedules, the `scheduling` tool group, their
  system-prompt line and their transcript formatters. `remind` is unchanged.

efbfec6d01 config PRs: remove the hive's config-PR webhook, poll and core merge
An operator's merge on the forge deploys a config PR through
swarm-controller's DeployRequest{rev}. The hive-side path that queued a
MergeConfigPr approval and merged the PR as `core` goes:

- the `/webhook/config-pr` receiver, its HMAC secret, the WebhookRegister
  boot node and the org-hook registration; the hive vhost's `/webhook/`
  location
- the 5-minute config-PR poll
- ApprovalKind::MergeConfigPr, its dashboard card, and the deploy DAG it
  drove (DeployWindow, MergeVerify, DeployApply, FinalizeDeploy,
  DeployTail), with verify_commit, the two-phase meta deploy, rollback
  refs, the PR-failure comment and forge/pr_merge.rs
- `fetched_sha`, `sha_short`/`pr_number` on approval events, and
  `sha`/`tag` on HelperEvent::ApprovalResolved: only the merge path set
  them

`config_repo`, `merged_pr_for_commit` and `post_pr_comment` move to
forge/pr_comment.rs for the merged-rev deploy's refusal comment.
Approvals v5 drops stored `merge_config_pr` rows; a test reopens a v4
database holding them.

d94bc2188d topology: drop the parent field and the hierarchy it fed
`topology.json` was a map of `name -> parent | null`, and that value fed
the whole agent hierarchy: `<parent>` / `<children>` recipient sentinels,
the reparenting API (CLI verb, wire verb, dashboard endpoints, DAG node),
the dashboard tree, the rebuild depth sort, and an unconditional
bind-mount grant giving every agent RW on its direct children's state.

Per the operator's ruling the field goes, and with it all of the above.
The file survives as what remains once the value is gone: the roster of
agent names, which is the set `ManageRootAgent` grants mounts over. It is
now a JSON array; `read` still accepts the old map shape and keeps its
keys, so a hive that upgrades across this does not blank its roster (and
so no capability holder loses its mounts for the length of that window).

Two sites kept their behaviour under a different recipient rather than
losing it. Both addressed `<parent>`, which the broker already resolved to
`operator` for a root agent, and every agent is now what that fallback
called a root:

- the harness's turn-failure / plugin-failure notification
  (`Surface::send_to_parent` -> `send_to_operator`), and
- the send allow-list's always-permitted escape hatch, so an agent with a
  restrictive allow-list still has a way to say it is stuck.

What is NOT preserved, deliberately: an agent with no capability no longer
sees any other agent's dirs. `ManageRootAgent`'s own grant is unchanged --
still every agent in the roster, still state RW + config RO, still no
`harness`.

The dashboard's reparenting control (the M0V3 picker) is deleted with its
CSS. The tree rendering that reads `ContainerView.parent` is left for the
frontend owner -- it degrades to a flat list with the field gone.

5b27aa18c2 docs: repoint eighteen pointers whose section no longer exists
`check-doc-refs.sh` resolves the PATH half of a `docs/x.md::Section`
pointer and stops there. The section half rots the same way, and more
quietly: the file still exists, so every path-shaped check stays green
while the pointer names nothing.

Eighteen sites, five distinct pointers, each retargeted at a heading
verified to exist rather than at the nearest plausible one:

  docs/web-ui.md::Container row
    -> docs/web-ui/dashboard.md::Container row
  docs/web-ui.md::Shared terminal pane
    -> docs/web-ui/shape.md::Shared terminal pane
      Both sections moved out when docs/web-ui.md became a two-heading
      index. The path still resolves, which is exactly why nothing
      caught them.

  approvals.md::Helper events to the manager
    -> approvals.md::Helper events to the submitting agent
      Renamed with the manager special-casing removal; the pointer kept
      the old vocabulary.

  approvals.md::Migration from the pre-tag
    -> approvals.md::Startup migrations (older hosts)
      Same content, including the HIVE_SKIP_META_MIGRATION kill switch
      the citing comment names.

  agent-hierarchy.md::Current state
    -> ::Where the tree lives   (topology.rs, container_view.rs)
    -> ::Reparenting            (topology.rs's set_parent, host-sock)
      Split by what each site actually asks for rather than repointed
      uniformly: two want the format and the source-of-truth rule, two
      want the reparenting validation.

Three known-dead pointers are deliberately left alone:

  * `docs/integrations/forge.md::Sources` sits on a line PR #3927
    rewrites; fixing it here would conflict for no gain.
  * `docs/web-ui/shape.md::One unified channel` names real text that is
    bold inline rather than a heading — which of those counts as
    resolvable is the open question on #3922.
  * `persistence.md::Harness state files` should point at a heading
    whose own text contains backticks, and the backticked-pointer form
    cannot nest them. That is a limit of the convention, not a typo.

Comments only; no behaviour change. Refs #3922.

07b62612b0 docs: restructure into topic subdirectories, collapse duplicated index
Per mara's go-ahead on hyperhive#3902 ("getting started is good, but
terminal rendering does not go in there i think"):

Moved 21 top-level docs/*.md files into 7 new topic subdirectories
(existing web-ui/, turn-loop/, swarm/, tools/, crates/ untouched):
  getting-started/  setup.md
  agent-lifecycle/  agent-hierarchy.md, approvals.md, persistence.md
  trust-boundary/   boundary.md, security.md
  integrations/     forge.md, matrix.md, github.md, knowledge.md
  networking/       gateway.md, network.md, snapshot-store.md
  scheduler/        jobq.md, coordinator.md, ci.md, observability.md
  process/          conventions.md, gotchas.md, pr-review-gate.md
  web-ui/ terminal-rendering.md (moved into the EXISTING dir,
                    per mara's correction to the original getting-started
                    guess -- it's UI implementation detail, not onboarding)

The physical layout now matches docs/README.md's own topical headers,
which already amounted to this taxonomy -- see the scoping comment on
the issue for the two findings that motivated this (a genuine
duplication between CLAUDE.md's old "Reading paths" list and
docs/README.md's grouped one, since drifted out of sync with each
other; and the flat layout not matching the grouping we already had).

Fixed every cross-reference this moved across the whole repo (~120
files: docs/ internal links at every depth, Rust doc comments, nix
module option docs, crate READMEs) -- verified two ways: a grep sweep
confirming zero remaining references to any old path, and a script
that resolves every markdown link in docs/**/*.md + CLAUDE.md +
README.md against the filesystem and reports anything that doesn't
exist (zero broken links).

Collapsed CLAUDE.md's "Reading paths" section (the duplicate) down to
a pointer at docs/README.md, now the single index. Rewrote
docs/README.md itself to use the new subdirectory paths and added the
one doc it was missing that CLAUDE.md's old copy had (pr-review-gate.md).

Classified all 22 docs/*.md files first via a haiku subagent (mara's
suggestion) on two axes -- proposed grouping and operator-vs-
implementation focus -- before finalizing the taxonomy; spot-checked
the report and found internal inconsistencies (its classification
table disagreed with its own summary section for a few files), so this
taxonomy is my original proposal + the one correction mara gave
directly, not a blind application of the subagent's table. The
operator-focus data it gathered is still useful for a follow-up
content pass (docs skewing 'mixed' rather than pure operator-facing),
not addressed in this PR -- structure only.

nix fmt clean, both pre-push lints clean.

27932ec631 types: let nix own the reserved-name blacklist
One list, in nix/reserved-names.nix, handed to everything that needs it
as HIVE_RESERVED_NAMES. Keeping it current becomes a config change
rather than a rebuild, and hive names and agent names -- one namespace
going forward -- are checked against the same file: swarm-otel.nix's
hand-written reservedOwners is gone.

Whitespace-separated rather than JSON, deliberately, unlike the
structured env vars beside it. Every entry is an Ident ([a-z0-9-]), so
whitespace cannot occur inside a name and the encoding is provably
lossless; JSON would mean either a parser dependency in a crate whose
purpose is to have none, or a copy of the parse in every consumer.

An UNSET variable is not "nothing is reserved". Both creation sites log
an error and return a warning saying the check did not run, so a
misconfigured deployment says so instead of silently accepting every
name. A blank value folds into unset: nix always renders a non-empty
list, so present-but-empty is a rendering fault, not a declaration.

Two guards whose subject moved out of their own file now assert their
own case is still in it, because a guard that can be retired by an edit
elsewhere is not a guard:

- swarm-otel.nix asserts reserved-names.nix still contains its
  swarmTierName.
- hive-sh4re's sentinel drift test PANICS when the variable is missing
  rather than skipping -- a drift test that quietly does nothing still
  reports green. checks.nix and devshell.nix both export it so CI and a
  local cargo test agree. Verified as a pair: with the variable set, 8
  tests pass; with it unset, exactly the 4 drift tests fail and the
  unrelated ones still pass.

5202e5c5ba types: reserve the protocol names an agent must not be called
An agent's name was checked for shape and never for meaning:
`Ident::parse` is 1-63 chars of [a-z0-9-] and there was no reserved-name
list anywhere in the tree. So an agent could be called `operator`,
`forge` or `todo` -- names the message layer already produces as a
sender -- and a wake from that component became indistinguishable, at the
broker, from a message sent by the agent.

Adds `RESERVED_NAMES` + `is_reserved_name` to `hive-types`, the zero-dep
leaf both `hive-c0re` and `swarm-controller` already depend on, so
neither grows a dependency to use it.

Every entry is a value some component actually produces as a message
`from`/`to`, taken from `hive-sh4re`'s own sentinel constants rather than
guessed: operator, system, reminder, forge, scheduled, todo, compact,
graceful-stop. Two sentinels are deliberately absent -- `<parent>` and
`<children>` are unreachable as agent names because the charset rejects
them, and `ruth` is a real agent, so wanting that name is a name being
*taken*, which the roster answers.

Deliberately not enforced inside `Ident::parse`: parsing runs on every
read of an already-created name, so rejecting there would make existing
agents unreadable rather than un-creatable -- and it would be a refusal,
which is a stronger action than the warning this is used for today.

`create_agent` now warns on both halves -- a reserved name, and a name
that is also a hive in the roster -- and does not refuse. The warnings
ride on `CreateAgentResponse` rather than only the daemon's log, because
the person who can still fix the name in one keystroke is holding the
response, not reading the journal. `skip_serializing_if` keeps the
no-warning JSON byte-identical to before, so this is a non-breaking first
step toward refusing later.

`hive-sh4re` gains a drift test tying its sentinel constants to the list:
two crates that cannot import each other's intent now fail loudly if a
sentinel is added without being reserved. Mutation-verified -- forcing
the predicate false, forcing it true, and dropping a single entry each
turn a different test red.
