8c222f8910 hive-c0re: a refused rev is an error log + Grafana alert, not a PR comment
A deploy request whose rev hive-c0re refuses (fetch failed, or the rev
does not descend from applied/main) no longer tries to comment on the PR
that merged it. Off the forge host that attempt always failed with "forge
core token absent". The refusal now logs at ERROR, message unchanged, and
the new `hh-config-rev-refused` Grafana rule fires on it.

Deleted with the posting: actions::post_rev_deploy_failure_to_pr, the
whole forge/pr_comment.rs (config_repo, merged_pr_for_commit,
post_pr_comment), and ReaderToken::value, whose only caller was
merged_pr_for_commit.

1726ad3bad hive-c0re: bound the forge readiness probe
wait_until_ready only checked READY_TIMEOUT between polls, so a half-open forge
hung the first get_version forever. The probe now goes through forge::bounded; a
timeout counts as not-ready and falls into the existing retry/deadline path.

2b0154be28 hive-c0re: bound forge API calls so a stalled forge can't wedge the swarm event loop
forgejo-api 0.11.1 takes no caller-built client, so a request to a forge that
accepts the connection and never answers waited forever. Every forge::api()
call site now goes through forge::bounded, a 10s timeout that errors naming the
operation. The PR-comment calls run inline in swarm_status::drain_swarm_events,
the one task serving both swarm subscriptions, so a stalled forge stopped both.

f53ccb2740 hive-c0re: stop silently dropping errors on credential/marker writes
Add secret_file::write_secret_0600: create the parent dir, write a
sibling temp file opened create_new + mode 0600, sync_all, rename over
the target. Use it for the matrix sender token, the space/chat room-id
files and the forge core token, which previously wrote at the process
umask, then chmod-ed with the error discarded, and dropped the
create_dir_all error. agent_identity::write_identity uses it too
instead of its own open-0600 loop; that loop opened with create +
truncate, so a pre-existing file kept its old mode.

Also log instead of discarding:
- the manager's applied-rev marker write in auto_update
- the knowledge `git remote set-url` scrub (spawn error and non-zero
exit; the exit status only, since stderr may echo the old URL)
- the parent create_dir_all in paths::move_if_legacy

1ca758daa7 swarm-controller grants internal/docs read to agents; hives stop doing it
Agent creation gains a GrantDocsRead node (after CreateForgeUser) that adds
the agent as a read collaborator on internal/docs, and forge::docs_grant
runs a pass over the store's hive-agent-* roster at start and every five
minutes that queues the same job for any agent that is not one. The pass only
adds; an unreadable roster or forge read changes nothing.

hive-c0re drops shared_docs_access, its call in sync_agent, and the
SHARED_ORG / SHARED_DOCS_REPO constants only it used.

21e3f626f4 hives read the forge through their own swarm-minted read account
swarm-controller now gives every hive in its directory a forge account,
`hive-<hive>`, in a new `hives` team of `agent-configs` (read on
`repo.code` + `repo.pulls`, every repo including later ones), and mints
one `read:repository` token named `swarm-hive` into the store at
`swarm/hives/<hive>/forge/token`. A five-minute pass queues a
`MintHiveForgeToken` node for a hive whose token is missing or no longer
matches the forge, whose account is missing, or whose team membership or
team settings drifted. A hive whose store or forge read failed is never
queued, so an outage cannot rotate every hive's token. The token decision
reuses `agent_token`'s classifier and replace step, now parameterised by
name and scopes.

An agent may be called `hive-<x>`. The hive account is created with
`hive-<hive>@hives.hyperhive.local` and the job refuses an account by that
name with any other email before it locks it down, adds it to the team or
mints for it; agent creation already refuses the hive's account by the
same email check.

The controller's bao policy gains the `swarm/hives/+/forge/token` leaf
(create/read/update), pinned by the bao-grants module-eval assertion. The
hive's own policy already reads `swarm/hives/<hive>/*`.

hive-c0re reads that token through `forge::reader_token()`, falling back
to the core token file when the store holds none, and uses it for:
fetching a merged config rev (swarm deploy, `hivectl forge
reconcile-config`), reseeding a purged `applied`, the meta-update
cascade's fast-forward, seeding `proposed` on first deploy, and finding
the PR that merged a refused rev. Those reads no longer require a local
`hive-forge` container. The `/knowledge` clone moves from the forge
host's boot sweep into the knowledge pull, so every hive clones it.

Posting the deploy-failure comment on that PR stays on the core token.

c8472966e9 hive-c0re: drop the remaining "mirror" wording for the removed push
The reseed bail text, the `ensure_all` doc, the forge module doc and a
rebuild comment still described hive-c0re mirroring the applied config to
`agent-configs/<name>`. State what the code does now; scope the
config-changes doc to swarm-created agents and say what a legacy agent

456a4511ab hive-c0re: stop mirroring deploy tags to the forge (drop push_config)
Per the operator's ruling on #4851 (option (a)): the hive holds no
write identity on the forge, so the `push_config` mirror of an applied
repo's tags and `main` to `agent-configs/<name>` is removed, along with
its `run_config_push` helper and its call in `sync_agent`.

Docs and comments that described the mirror now say that forge `main`
advances only by an operator's merge.

e2da5509ae swarm-ci-runner: run the forge CI runner as a swarm service
The CI runner was a hive deploy module: hive-ci.nix ran it beside the
forge, and hive-c0re registered it through the forge's admin API with
the `core` token, writing the registration token through hive-priv. It
could only run on the forge's host, and it kept an admin-scoped forge
token on a hive.

Now the runner is a swarm service:

- swarm-ci-runner.nix runs it in a `swarm-ci-runner` container on the
host that sets deploy.ci-runner.enable, reaching the forge by name.
It needs neither the forge nor a hive there.
- swarm-controller reads the forge's runner registration token with its
own admin token and writes it to swarm/services/ci-runner/
registration-token whenever it differs from what it last wrote. Its
grant on that path is create/update only.
- The runner's own leaf (CN swarm-ci-runner, role and policy of the same
name) reads that one path. swarm-bao-ci-runner.service fetches it into
the runner's EnvironmentFile and restarts the runner on a change.
- A pre-start step removes .runner when the daemon's journal since its
last start holds Forgejo's "unregistered runner" refusal (the daemon
exits 1 when its startup Declare gets it), or when .runner names
another forge address; upstream's pre-start step then registers again.

Removed from the hive side: hive-ci.nix and deploy.forgejo.ci.*,
forge/ci_runner.rs, PrivRequest::RegisterCiRunner and its handler and
ReadWritePaths entry, InfraContainer::Ci with hivectl's --ci and the
dashboard's hive-ci journal entry, HYPERHIVE_FORGE_CI_ENABLED, and
read:admin,write:admin from CORE_TOKEN_SCOPES: nothing else in hive-c0re
calls /api/v1/admin/*. The old ci options, under both swarm.forge.ci and
deploy.forgejo.ci, now fail evaluation naming deploy.ci-runner.

The forge's half of the old ci.enable, the actions/checkout mirror and
DEFAULT_ACTIONS_URL = self, is deploy.forgejo.actionsFromSelf.

58ef299c3c hive-c0re: remove the approvals subsystem
Nothing submits an approval since scheduled prompts left hive-c0re, so the
queue only acted on rows written before that.

- hive-c0re: the approvals store and its migrations, approve/deny/
resolve actions, the ResolveApproval tail node (and meta_update's
approval_id), POST /api/approve and /api/deny, the approval views on
/api/state, the approval_resolved dashboard event, the orphan GC,
destroy/purge failing pending rows, and the approval rows in
get_loose_ends. DestroyBookkeeping loses the purge flag, which only
  worded the approval-failure note.
- The CancelLooseEnd request goes: its only hive-c0re arm was the
approval one. cancel_loose_end handles reminder and todo in the harness.
- Wire types: hive_sh4re::approvals (ReminderStats moves to inbox),
  LooseEnd::Approval, CancelLooseEndKind, HelperEvent::ApprovalResolved,
  ToolGroup::Approvals, HostRequest::{Pending,Approve,Deny} and
HostResponse.approvals. HOST_SOCK_VERSION goes to 2 for the removed
  field.
- hivectl loses the approvals verbs; docs/tools/hivectl-cli.md is
  regenerated.
- The system prompt drops approval_resolved.
- The approvals table stays in broker.sqlite, unread, the same way the
operator_questions table was left when its store was removed.
- An `approvals` entry still in tool-groups.json is skipped with a warning
by ToolGroup::parse_list, like any unknown group.
- docs/agent-lifecycle/approvals.md is renamed to config-changes.md so the
code's doc pointers resolve; its content follows in the docs commit.

1de8a01936 hive-c0re: drop the spawn-time forge user-email, repo-creation and config-repo passes
sync_agent no longer runs ensure_user_email, ensure_repo_creation_disabled
or ensure_config_repo. swarm-controller does all three before any deploy:
CreateForgeUser sets the account email and max_repo_creation = 0 (and the
agent_token backfill re-runs CreateForgeUser), CreateRepo creates and
protects agent-configs/<agent>, AddRepoMember grants the agent write on it,
and InitAgentConfigRepo seeds it.

What goes with them: the email-aligned and repo-creation-disabled marker
paths and the legacy email-marker relocate, sparse_edit_user_option,
is_forbidden and its tests, and ensure_org_repo. A config repo deleted by
hand is no longer recreated on the next spawn; the operator recreates the
agent at swarm level. Comments in swarm-controller that described the
removed hive passes now state only the current behaviour.

56f592d525 config PRs: each hive removes its own stale config-PR org hook on boot
The hook a hive registered on the agent-configs org points at its
/webhook/config-pr route, which no hive serves any more, so every
config-repo event fails delivery to it. The forge sweep now deletes it.

Only a hook whose URL equals this hive's own exactly is removed. The same
path on another base belongs to another hive and is left alone. A forge
error is logged and boot continues; once the hook is gone the step is a
no-op.

0cee0382e9 config repos: drop the hive's branch-protection edit and core from the merge gate
Folds in #4853: hive-c0re no longer writes branch protection on
`agent-configs` repos (apply_config_repo_branch_protection,
config_repo_protection_edit, record_branch_protection_result and the
now-unused main_branch_protection_option go). swarm-controller's
CreateRepo rule and its forge-objects convergence own `main`'s gate.

Nothing merges into config `main` as `core` any more, so the converged
rule's merge user list is empty. `push_config` still pushes `main` as
core, through push rights, not the merge whitelist.

efbfec6d01 config PRs: remove the hive's config-PR webhook, poll and core merge
An operator's merge on the forge deploys a config PR through
swarm-controller's DeployRequest{rev}. The hive-side path that queued a
MergeConfigPr approval and merged the PR as `core` goes:

- the `/webhook/config-pr` receiver, its HMAC secret, the WebhookRegister
boot node and the org-hook registration; the hive vhost's `/webhook/`
  location
- the 5-minute config-PR poll
- ApprovalKind::MergeConfigPr, its dashboard card, and the deploy DAG it
  drove (DeployWindow, MergeVerify, DeployApply, FinalizeDeploy,
DeployTail), with verify_commit, the two-phase meta deploy, rollback
refs, the PR-failure comment and forge/pr_merge.rs
- `fetched_sha`, `sha_short`/`pr_number` on approval events, and
`sha`/`tag` on HelperEvent::ApprovalResolved: only the merge path set
  them

`config_repo`, `merged_pr_for_commit` and `post_pr_comment` move to
forge/pr_comment.rs for the merged-rev deploy's refusal comment.
Approvals v5 drops stored `merge_config_pr` rows; a test reopens a v4
database holding them.

f1c695c212 config PRs: an operator's Forgejo merge deploys the merged rev
A config PR merged in the Forgejo UI changed nothing on the hive: the
hive's webhook ignores `closed`, its poll then cancels the dashboard
card, and `applied/main` stays where it was.

swarm-controller reads `merged`/`merge_commit_sha` off the
`pull_request` delivery it already receives for `agent-configs`, finds
the hive placing the agent by scanning every hive's wanted state (the
scan `declarations_elsewhere` already ran, factored out), and queues a
`TriggerDeploy` carrying the rev. Zero or several claimants deploy
nothing and log the claimants.

`DeployRequest` gains `rev: Option<String>` with `serde(default)`, so
rev-less payloads from either side keep decoding.

hive-c0re, given a rev for an agent it runs: a no-op when
`applied/main` already is the rev (a dashboard merge deploys its own
PR); otherwise it fetches the forge `main` with the core token,
requires the rev to descend from `applied/main` (the ancestry gate,
factored out of `run_deploy_merge_verify`), fast-forwards by CAS and
queues the usual relocking rebuild. No eval-verify on this path, per
mara (#4850 c90075). A refusal is commented on the PR that merged the
rev, found by commit.

swarm-controller's forge-objects pass converges every config repo's
`main` rule to merge whitelist `operators` + `core` and approval
whitelist `operators`. The hive's boot PATCH stops forcing
`enable_approvals_whitelist` off, so the two do not fight.

7d217f8267 remove the create_repo agent tool
mara ruled on #4849 (c88934): "remove create_repo tool". The tool ran in
hive-c0re with the hive's core token, so it only ever worked for agents
on the hive that runs the forge.

Removed:

- the create_repo MCP tool and CreateRepoArgs (hive-agent-mcp)
- wire variants Request::CreateRepo and Response::RepoCreated
  (hive-core-agent-sock)
- hive-c0re's handle_create_repo, its valid_repo_name check and the
  dispatch arm
- forge::create_agent_repo and apply_operator_branch_protection, which
had no other caller, plus AGENTS_ORG and OPERATORS_TEAM, whose only
  users they were
- the tool's docs (docs/tools/forge.md repo management, docs/turn-loop/
mcp.md, the conventions tool-group table) and the doc comments that
named it (hive-sock-client's response timeout, ensure_repo_creation_
disabled, the security doc's merge-gate bullet)

ToolGroup::Forge is kept with no tools, the same way b88a5b24 kept
Lifecycle, so existing meta/capabilities.json grants still parse.

Forge state is untouched: existing agents/* repos keep their collaborators
and operators-team branch protection. The swarm-controller's own
create_repo (config-org repos) is a different path and is unchanged.

bfd8189900 hive-c0re: stop reporting refused invites as success; re-register the config-PR hook when its secret changes
invite_user_id mapped every 403 M_FORBIDDEN to Ok(()). The membership
pre-check already skips invited/joined users, so the 403s that reach the
POST are mostly real refusals (banned target, sender without power),
including `hivectl matrix invite`. A 403 is now success only when a
membership re-read shows the user invited or joined; otherwise it is an
error carrying the status and body.

admin_room_send_and_poll read the send response's event_id with
unwrap_or_default() and, when it was missing, walked every recent event
unanchored, so an older bot reply (an earlier reset password) could be
returned as this command's result. A send response without an event_id
is now an error.

run_destroy_bookkeeping discarded fail_pending_for_agent's error; it now
warns like its neighbouring steps.

ensure_config_pr_webhook returned as soon as a hook with the target URL
existed, so a regenerated webhook-secret never reached Forgejo and every
config-PR delivery failed HMAC until the 5-minute poll caught up.
Forgejo's edit-hook API ignores `secret` and never returns it, so the
SHA-256 of the secret last registered is recorded at
forge/config-pr-webhook-secret-sha256; when it doesn't match, the
same-URL hook is deleted and recreated. The paths.rs doc claiming
re-registration on change now describes this.

7b1fe5f9d3 hive-sock-client, web proxy, HTTP clients: bound connect and response waits
hive-sock-client: each attempt now bounds connect (5s), write (10s) and
the wait for the response (60s by default). The response bound is per
call through the new `request_within`, which hive-agent's serve-loop
`Recv` uses with its 180s long-poll plus 30s headroom. A response
timeout is terminal rather than retried: the server holds the request,
so a retry re-sends something it may still act on and multiplies the
wait by the backoff schedule.

Outbound HTTP: the matrix login/whoami clients in swarm-controller and
hive-c0re's dashboard (5s connect, 30s request), the authelia-bridge
client (5s/30s; ensuring an identity runs an argon2 hash first) and the
ci-runner forge calls (5s/15s, config_pr_poll's forge budget) get a
connect_timeout and a request timeout. Timeout errors name the bound
that fired.

hive-agent's unix-socket extra web proxy bounds the connect (5s) and
the wait for the response head (30s, the http sibling's budget); the
body read stays unbounded.

20419ccd41 swarm-controller: own the swarm-wide forge objects; hive-c0re stops creating them
The orgs agent-configs/internal/agents (plus mirror owners), the
operators team in agents and agent-configs, the pull-mirrors,
internal/docs, internal/knowledge (public, README-seeded) and the
agent-configs org avatar are one set per forge. hive-c0re ensured them in
its boot sweep, as the core admin, and only on the hive co-located with
the forge container.

swarm-controller now reconciles them at start and every 5 minutes
(forge/objects.rs: observe -> pure plan -> apply). A failed object logs
a warn line plus a pass summary and is retried next tick. create_repo
ensures the agent-configs org and its operators team first, so a config
repo's merge gate never depends on the periodic pass having run.

hive-c0re drops ensure_org, SEEDED_ORGS, ensure_mirrors/ensure_mirror_repo,
ensure_operators_team, ensure_shared_docs_repo, ensure_knowledge_repo/
set_repo_public, seed_readme, ensure_config_org_avatar and the one-shot
knowledge::remove_webhook cleanup, with their now-unused helpers.

nix: the mirror list moves from the hive-c0re unit

declared on a host that runs no controller. c0re.orgAvatarPng is renamed

2c066cc871 hive-c0re: pin the two security boundaries that had no test
`is_forbidden` and the webhook-secret load/regenerate path were the last two
entries on the shortlist in hyperhive/hyperhive#3950; the other three landed in
hyperhive/hyperhive#4650. Both are classification logic whose failure mode is
silence, which is why they are worth a test rather than a coverage line.

`is_forbidden` gates the arms that tell an operator a Forgejo admin PATCH was
refused for want of a scope, and which credential to delete and re-mint to fix
it. One of those PATCHes is `ensure_repo_creation_disabled` — the lockdown that
stops an agent creating a repo it owns and self-merging in it. Two tests: a 403
is recognised in both shapes the typed client produces (the spec-listed
`Forbidden` kind and the bare `UnexpectedStatusCode`), and nothing else is —
not a 401, whose remedy is the automatic re-mint one function down, and not a
transport error that never reached the forge at all.

`load_or_generate` grows the path-taking half `load_or_generate_at`, the same
seam `swarm-controller`'s `webhook::load_or_generate_at` already has and for
the same stated reason. Three tests over it: a valid stored secret is returned
verbatim and never rotated (the newline this module writes itself makes the
trim load-bearing, not defensive); a malformed one is replaced by a secret that
reaches *disk*, not just the caller, and is then stable; and each near miss —
empty, whitespace, 63 chars, 65 chars, right length with a non-hex char — is
refused. That last one is the security case: `Hmac::new_from_slice` accepts a
key of any length, empty included, so a relaxed check fails nowhere and just
keys every signature off a guessable value.

Every test was confirmed able to fail: six mutations of the code under test,
each watched red, then reverted. The two halves of the validity check and the
two arms of `is_forbidden` were broken separately, so neither test passes on
one arm alone.

c45d679a32 hive-c0re: fast-forward applied/<name>/main too, not just the one-shot relock
argus + mara (PR #4339 review): the previous commit's per-agent lock
relock is a one-shot effect on the single rebuild the cascade triggers
- applied/<name> never moves, so the next relock=true rebuild trigger
(the boot sweep, most notably) re-locks against applied/<name> and
reverts straight back to whatever it was stuck on. The fix didn't
outlive the transaction it ran in.

New forge::fast_forward_applied_main(name), sibling to the existing
reseed-only fetch_config_main_into_applied: for an applied repo that
already has a .git and just needs to catch up, force-set rather than
fast-forward-gated since there's no PR to review on this path either.
Called per cascade agent right after the relock, best-effort so one
unreachable agent repo doesn't block the others.

Once applied/<name>/main has actually moved, lock_update_for_rebuild's
override (always reads current applied/<name>/main, no ?rev pin)
naturally stays in sync on any later relock=true rebuild instead of
reverting.

                                       -> nix/packages/frontend.nix

The two `limits.rs` comments are a matched pair: each names the other's
old path, so the "keep in sync" instruction they exist to carry pointed
both ways at nothing.

Where a flat module became a directory the target is the file that
declares the named thing, not `default.nix` by reflex — the
`preBuildAgentTemplates` option is declared in `options.nix`, and the
DHCP pool that sentence is about lives in `dnsmasq.nix`.

Comments only; no behaviour change. Refs #3923, which is about whether a
gate should cover this class at all — that question is unanswered and
this does not close it.

07b62612b0 docs: restructure into topic subdirectories, collapse duplicated index
Per mara's go-ahead on hyperhive#3902 ("getting started is good, but
terminal rendering does not go in there i think"):

Moved 21 top-level docs/*.md files into 7 new topic subdirectories
(existing web-ui/, turn-loop/, swarm/, tools/, crates/ untouched):
  getting-started/ setup.md
  agent-lifecycle/ agent-hierarchy.md, approvals.md, persistence.md
  trust-boundary/ boundary.md, security.md
  integrations/ forge.md, matrix.md, github.md, knowledge.md
  networking/ gateway.md, network.md, snapshot-store.md
  scheduler/ jobq.md, coordinator.md, ci.md, observability.md
  process/ conventions.md, gotchas.md, pr-review-gate.md
web-ui/ terminal-rendering.md (moved into the EXISTING dir,
per mara's correction to the original getting-started
guess -- it's UI implementation detail, not onboarding)

The physical layout now matches docs/README.md's own topical headers,
which already amounted to this taxonomy -- see the scoping comment on
the issue for the two findings that motivated this (a genuine
duplication between CLAUDE.md's old "Reading paths" list and
docs/README.md's grouped one, since drifted out of sync with each
other; and the flat layout not matching the grouping we already had).

Fixed every cross-reference this moved across the whole repo (~120
files: docs/ internal links at every depth, Rust doc comments, nix
module option docs, crate READMEs) -- verified two ways: a grep sweep
confirming zero remaining references to any old path, and a script
that resolves every markdown link in docs/**/*.md + CLAUDE.md +
README.md against the filesystem and reports anything that doesn't
exist (zero broken links).

Collapsed CLAUDE.md's "Reading paths" section (the duplicate) down to
a pointer at docs/README.md, now the single index. Rewrote
docs/README.md itself to use the new subdirectory paths and added the
one doc it was missing that CLAUDE.md's old copy had (pr-review-gate.md).

Classified all 22 docs/*.md files first via a haiku subagent (mara's
suggestion) on two axes -- proposed grouping and operator-vs-
implementation focus -- before finalizing the taxonomy; spot-checked
the report and found internal inconsistencies (its classification
table disagreed with its own summary section for a few files), so this
taxonomy is my original proposal + the one correction mara gave
directly, not a blind application of the subagent's table. The
operator-focus data it gathered is still useful for a follow-up
content pass (docs skewing 'mixed' rather than pure operator-facing),
not addressed in this PR -- structure only.

nix fmt clean, both pre-push lints clean.

ccf9951e5d hive-c0re: seed an agent's proposed config from the forge when it exists
The swarm writes agent-configs/<agent> when it creates an agent, before
any hive is told to deploy it. setup_proposed authored a second copy of
those same bytes locally, so an agent's initial config had two sources
of truth, each unaware of the other and free to disagree. It now clones
that repo and falls back to the template only when there is nothing
there to take.

Preferred-source rather than a new-path-only variant because
provision_container is the Provision node for the swarm deploy and the
approval flow both, and cannot tell them apart. The approval flow
creates agent-configs/<agent> only after the first spawn
(forge_after_first_spawn), so it finds nothing and lands on the
template: the fallback becomes unreachable when hive-level create is
removed, rather than becoming something someone has to find and delete.

clone, not the neighbouring init+fetch. A failed fetch leaves an empty
.git behind, and that .git is exactly the byte setup_proposed reads to
decide whether seeding is still needed, so the fallback would have seen
a seeded repo. git removes a directory it created when a clone fails.
--branch main also makes an empty repo fail cleanly instead of cloning
to an unborn HEAD that would look seeded.

4423da4899 deploy: move the forge CI runner toggle out of swarm
Last of the swarm.* -> deploy.* moves for the enable-shaped options.
Whether this host also runs the CI runner is a per-machine decision, so
it was sitting in the namespace that has to be identical swarm-wide.

Renames all five options, not just enable: name, concurrency, labels and
package all describe the runner THIS host would run, and leaving them
behind would keep the original defect for four more options. One rename
entry each, because ci is a plain attrset rather than a submodule type,
so there is no parent path to rename in one go.

deploy.forgejo is the name deploy.nix's own header already specifies,
including this exact case -- "ci (does this host run the runner too) is
exactly that shape". It is the only entry with no enable: the forge is
not optional, it deploys with hyperhive itself, so running the runner is
the only deployment decision it has.

Also fixes hive-c0re/src/forge/ci_runner.rs, whose doc comment named
services.hyperhive.forge.ci.enable -- missing the swarm. segment, so it
had never resolved to a real option.

ca4b56c006 hive-c0re: stop naming nix implementation files in env-var prose
Six diagnostics and eight comments explained where an environment
variable comes from by naming the module file that sets it. The reader
of a panic is an operator, who does not edit that file; the reader of
the comment is already in the code. Neither can act on a filename, and
a filename in prose is checked by nothing.

Each one now names what the reader can act on: the option where there
is one (services.hyperhive.network.bridgeName, .bridgeIp,
.bridgePrefixLength, c0re.assets, c0re.orgAvatarPng), otherwise the
unit the process is expected to run under. Two of the six diagnostics
had a real option available; the other four have none, so they name
the unit rather than an invented path.

Four of them pointed at hive-c0re.nix, which is a directory. Two of
those spelled it nix/modules/hive-c0re.nix — a directory that does not
exist either, and one of the two was wrapped across a line break, so
no grep for the whole path could have found it. That is the argument
for the change: prose naming a file rots silently because nothing
compiles it.

Scope here is the four files around those diagnostics. The same
pattern still matches elsewhere in the tree; that sweep follows.

ebb4eea691 hive-c0re: re-register the CI runner when the forge address changes
act_runner records the --instance URL it was registered with and reads the
forge address from .runner and nowhere else: the nix option reaches only
`register`, and upstream re-registers on a changed token or labels, never on
a changed URL. Our own precond short-circuits on .runner existing, and
runner_valid asks whether the runner id still exists -- which after a rename
it does. So changing swarm.forge.domain left the runner dialling a name its
container's derived extraHosts no longer resolves, with every check green.
The symptom is CI going quiet rather than anything failing.

Compare the persisted address against the configured base as part of the
same early return. A disagreement mints a fresh registration token, which
changes the token hash upstream already keys on, so upstream removes .runner
and re-registers against the current --instance. This module never writes or
deletes that file; the deletion stays with the script that owns registration.

The comparison is host-only and lenient on purpose. Both sides render from
one nix expression -- http://${swarm.forge.domain}, reaching the runner as
instances.hive.url and this daemon as HIVE_FORGE_URL -- so they cannot drift,
while scheme, port and trailing slash are exactly the cosmetic differences
that would otherwise re-register on every boot. Unreadable, missing or
unparseable input keeps the existing credentials, matching runner_valid's
treatment of a transport error: only a positive disagreement counts.

84125b0806 docs(#3255): correct the reaper invariant the swarm webhook test guards
The test's doc named knowledge::ensure_webhook as one of two hive-side
reapers. That function is gone; its replacement matches the full URL and
cannot touch another hive's hook.

The assertion arm stays. The hazard is not what this repository's source
says, it is what is deployed — a hive still running the previous version
reaps by suffix until it is upgraded, so the arm guards the transition
rather than a current code path. Recorded when to drop it.

e15c499a31 fix(#3245): resolve the remaining intra-doc links in hive-c0re
Takes the crate from 26 rustdoc warnings to 1, on top of the ten in the
previous commit.

argus's review findings:
- agent_sockets.rs: [`write`] was still ambiguous (function vs macro).
  The previous change narrowed the qualifier and left the ambiguity;
  [`write()`] is what resolves it.
- forge/users.rs <hex> and stats/container_stats.rs <name>: unclosed
HTML tags in prose, now backticked.

The rest of the crate, so the count actually reaches zero:
- job_queue/mod.rs: Queue::graph_snapshot -> JobQueue::graph_snapshot
(there is no Queue type), and super::scheduler -> scheduler (mod.rs
*is* job_queue, so super:: pointed outside it)
- job_queue/resource.rs: NodeKind -> super::model::NodeKind
- matrix.rs: password_path(name) -> password_path; and
  forge::provision_user_token -> crate::forge::provision_user_token.
Note the path has no `users` segment: forge/mod.rs declares `mod
users` private and re-exports it, so the canonical path comes from the
re-export rather than the directory tree.
- socket_server/lifecycle_handlers.rs: InfraContainer ->
  hive_priv_sock::InfraContainer
- stats/otel_metrics.rs: crate::meta::otel_config is a private fn no
path can name from another module, so it becomes prose
- main.rs: redundant explicit link target dropped

coordinator.rs:405 (CrashWatchGuard) is deliberately untouched: #3244
deletes that doc block, so fixing it here would conflict with an open PR
and repair a symbol that is about to stop existing.

517acc9f33 fix(#3245): resolve broken intra-doc links in hive-c0re
Remove or fix broken documentation links that accumulate silently:
- container_view.rs: HiveEnv reference
- forge/mod.rs: READY_TIMEOUT and webhook handler links
- workers/knowledge.rs: webhook handler link
- job_queue/model.rs: Claim::deps and WireNode::data references
- stats/hive_stats.rs: read_skill_breakdown reference
- stores/audit_log.rs: global() reference
- workers/agent_sockets.rs: ambiguous agent_sockets::write reference
- coordinator.rs: systemd.services.<harness> formatting
- resource_limits.rs: ambiguous write/read references

Some broken links were to deleted functions/types; these are replaced
with prose descriptions. Others referenced items outside this crate or
were private; these are replaced with plain text references or qualified
paths as appropriate.

e525dcb6d4 agent icon: 404 when unconfigured, client-side fallback
hive_sh4re::assets::branding_svg() resolved a server-side default
icon at runtime from HIVE_ASSETS_DIR — the only consumer was
serve_icon(), which fell back to it whenever the agent had no
`hyperhive.icon` override. Removed both the fallback and the
function: serve_icon() now 404s when /etc/hyperhive/icon.svg is
absent, and the per-agent web UI (app.js) picks up the existing
dashboard swarm.js pattern — swap the <img> src to the
frontend-bundled /favicon.svg on load failure, guarded against
looping if the fallback itself 404s.

Updated the doc/comment claims that said the server always returns
an image (docs/web-ui/agent.md, nix/agent-modules/default.nix, the
hive-c0re/forge/users.rs comment referencing the old shared-asset
set). forge-avatar-sync and the matrix avatar sync are unaffected —
both are gated on hyperhive.icon != null and never depended on the
removed fallback.

3e388d2d46 move avatar-PNG helpers to hive-core, configurable org avatar
core_avatar_png() and config_org_avatar_png() were hive-sh4re
functions used only by hive-c0re::forge::users. Move them there as
private path-resolution helpers (core_avatar_png_path,
config_org_avatar_png_path), following the existing
std::env::var(...).expect(...) required-env-var style used elsewhere
in hive-c0re.

The org avatar is now independently configurable:
services.hyperhive.c0re.orgAvatarPng (nullable path, default null)
lets an operator override just the agent-configs org avatar PNG
without replacing the whole assets package. Wired via a new
HIVE_ORG_AVATAR_PNG env var that falls back to the bundled PNG when
the option is unset. The core avatar stays under HIVE_ASSETS_DIR,
unchanged.

44572d1e1a fix(#2911): keep the forge token out of argv
`forge_git_url` spliced `core:<token>@` between scheme and authority, and
that URL is a process argument. `/proc/<pid>/cmdline` is mode 0444 —
world-readable — so the core admin token, which provisions every agent's
forge account, was published to any local user for the lifetime of each
git child. Seven call sites built such a URL.

The credential now travels in the environment instead:
`git_command_authed` sets `http.extraHeader` via `GIT_CONFIG_*`, which
git reads exactly like a config file, and `/proc/<pid>/environ` is 0400 —
owner-only. Same credential, materially smaller audience. The remote is a
plain `http://forge/<org>/<repo>.git`, and `forge_git_url` no longer takes
a token, so the old shape cannot be rebuilt by accident.

`knowledge`'s clone was the one place a credentialed URL was stored as a
named remote — git persists the clone URL into `.git/config`, so the
token sat on disk and every later `pull` authenticated from there. That
is the case `forge::repos::push_config` documents as forbidden ("the
tokenised URL ... deliberately never stored as a named remote"). `pull`
now rewrites `origin` to the plain URL first, which also scrubs the
persisted token from existing deployments, and authenticates from the
environment when a token is available. The repo is public, so the pull
still works without one.

Three call sites also stopped spawning `Command::new("git")` directly,
so they honour the `HYPERHIVE_GIT` path the NixOS module bakes in and
the `kill_on_drop` every other git spawn gets.

The two URL-shape tests now assert the *absence* of a credential, and a
new one decodes the header back to `core:<token>` — without that, a
malformed header would leave every forge operation silently anonymous
with the other assertions still green.

10285ff764 fix(#2911): name the forgejo admin verb in errors, not its arguments
forge_admin interpolated its whole argument vector into the error
context, and two callers pass a live operator password in that vector
(user create --password, user change-password). Any failure of those
commands wrote the password to hive-c0re's log in cleartext -- and the
likeliest trigger is forgejo rejecting a weak password, so the secret
got logged precisely because forgejo refused it.

Redacting the value after --password would repeat the bug the issue is
about: redact_password_line matched one keyword and a differently named
secret walked past it. A denylist fails open, silently, and the next
secret-bearing flag would leak until someone extended the list.

describe_forge_admin keeps the leading verb path and stops at the first
flag, so "user create --username iris --password ..." is reported as
"forgejo admin user create". The verbs are a closed set this crate
chooses itself; argument values never are, so a new flag is excluded by
construction. Nothing useful is lost -- the context says which operation
failed, and the underlying error already carries forgejo's own message
about why.

The same pattern in hive-priv is deliberately untouched: that crate runs
as root and the redactor's shape is still an open question on the issue.
This change holds under either answer.

1f8cfdabd9 fix(#2911): stop putting the minted forge token in an error string
mint_token interpolated forgejo's raw stdout into its anyhow context on
the parse-failure path, and on that call stdout carries the access token
that was just created. The happy path below it is careful to log only
the user and token names; the error path handed the secret over whole.

It fires exactly when forgejo's output format drifts, which is the same
drift that breaks extract_token in the first place -- so the "help me
debug this" context printed the secret it had failed to find.

Report the shape of the output (bytes, lines) instead of its contents.
That is what diagnoses a version drift anyway: you want to know forgejo
printed something with no token-shaped word in it, not the bytes.

Redaction at a logging call site does not cover the error path.
with_context and bail! are output channels too.

8ad3b8e987 fix(#2860): drop hive-c0re's hardcoded localhost forge fallbacks
Two sites, same class, different blast radius:

`forge_http_base()` fell back to `http://localhost:3000` when
`HIVE_FORGE_URL` was unset. The NixOS module sets that variable
unconditionally, so the fallback could only ever fire for a process
started outside the module — where guessing produces a confusing
"connection refused" far from its cause. It now panics saying exactly
that.

`forge_git_url()` had a second, nastier fallback: a base URL with no
`://` produced `http://core:<token>@localhost:3000/...`, sending a
*credentialed* git push at whatever happened to answer on the local
port. Split the credential-insertion half out as `git_url_with_base`,
which panics on a malformed base.

That split also lets the tests cover the shape without setting a
process-wide env var, which would race every other test in the binary.
Adds a case pinning that the scheme is carried through rather than
assumed — the old hardcoded `http://` would have silently downgraded a
TLS-fronted forge.

d4b9dc1ab9 fix(#2570): gate boot forge-provisioning behind a readiness poll
The boot provisioning pass (orgs, repos, teams, CI-runner token) all hits
the Forgejo API, but ensure_all only checked the container is *present*,
not that its HTTP is *listening*. A nixos-rebuild that restarts hive-forge
and hive-c0re together races: every ensure_* fired at a refused socket and
left a stale 'provisioning failed' banner that never cleared, since the
pass is one-shot. Poll GET /api/v1/version (unauthenticated) until it
answers, bounded at 1 minute, before provisioning; on timeout proceed
anyway so a genuinely-down forge still surfaces its real errors. Mirrors
the readiness-retry the gateway-nginx path already has.

06bc7e31c0 fix(#2289): wire sync_agent failures to dashboard warning banner
sync_agent() now returns bool (false if any step fails). ensure_all()
collects the names of agents whose sync failed and raises a single
set_boot_warning with the aggregated list:

forge: per-agent sync failed for: alice, bob (see journal for per-step
  detail)

The static_kind() leak is already used for per-org boot warnings in the
same file — the leak is bounded (one per hive-c0re boot, not per request)
so reusing it here is appropriate.

The rebuild call site in job_queue/exec.rs discards the bool return and
keeps its existing tracing::warn! lines, which is the right separation:
rebuilds are their own retry loop and don't need to post a persistent boot
warning.
