Rewrites the config-change flow around the forge merge and the
DeployRequest{rev} deploy, drops the MergeConfigPr approval, its deploy
DAG, the hive's `/webhook/` route and the `core` merge allowlist from
the docs, and states that operators join the `operators` team by hand.

An operator links an agent's GitHub personal access token in the swarm UI
(LinkGithubAccountForm, "link github account" on /agents). swarm-controller's
PUT /api/hives/{hive}/agents/{agent}/github-account stores it at
swarm/agents/<agent>/github-token (swarm_secret_client::github), a flat leaf
under the agent's prefix that the agent's existing read grant already covers:
no policy change, and no list grant, since there is one token per agent.
In the agent, hive-agent-github-token (oneshot + 2-minute timer, as the agent
user, under its own store certificate, ordered before hive-github-notify)
reads that path and writes <state>/github-token, 0600 and agent-owned, the
file the gh wrapper, git credential helper and hive-github-notify already
read. It replaces the file by rename only when the bytes changed and never
deletes it: a hive-written github-token stays until a token is linked in the
swarm UI. It is installed only with a store address and
services.hyperhive.agent.github.enable.
Removed: the dashboard's CR3D3NTIALS page (credentials.html/js/css, its
build entries and H0M3 tile; GITHUB was its only tab), hive-c0re's
dashboard/matrix_accounts.rs with GET/POST /api/github-account,
priv_client::write_agent_github_token, the host socket's
SetAgentGithubToken and `hivectl github set-token`, and hive-priv's
WriteAgentGithubToken with write_agent_state_file, its only caller gone.
Docs: integrations/github.md and swarm/ui.md describe the swarm path,
swarm/credentials.md gains the store-path row, and the hive UI docs,
hivectl docs and security.md's hive-priv table drop the removed pieces.

Per mara's review: the hive UI doc covers only what hive-c0re's pages
render. Removed the M4TR1X page section (the hive gateway redirects
/matrix/ to the swarm matrix client, which the swarm UI's quick links
open), the swarm-UI forge/matrix account-linking lines from the
CR3D3NTIALS section, the infra-services hivectl paragraph, and the H0M3
Matrix/Forge absence line. Added a single pointer to docs/swarm/ui.md,
and stated the account-linking and Matrix quick-link facts there.

The old FORGES tab wrote forge-<label>-token/forge-<label>.json directly;
the swarm-fetch unit that replaced it never deletes a pair for a label it
doesn't list, so those files keep being read by hive-forge -f <label>
until the operator links an account under the same label in the swarm UI,
which overwrites both files (nix/agent-modules/forge-accounts.nix:11-13,159-176).
hive-matrix-daemon removes undeclared matrix-token-<name> files on
every start (hive-matrix-mcp/src/main.rs:100), so there is no window
where an account linked through the old per-agent MATRIX tab keeps
working — it must be declared via matrixAccounts at swarm/agent
level.
- hivectl/README.md: split matrix.rs/github.rs into accurate per-module
lines (matrix.rs invites a matrix user to the hive Space/room,
cli.rs:289-296; it is not per-agent). Fixed the summary line's
'provisioning verbs' to name the actual verb groups left after the
facts pass.
- web-ui/README.md: the swarm/ui.md pointer no longer promises roster/
creating-agents/linking-accounts content that page doesn't have.
- Reverted the unrelated doesn't/does not drive-by at hivectl/README.md:5.
docs/web-ui/README.md now frames itself as the per-hive dashboard (host
approvals, container state, rebuild queue) and points to swarm/ui.md for
swarm-wide day to day, matching the README's swarm-first reframe.
Credentials page fixes: it has only a GitHub PAT tab now (2c7e586f
removed the FORGES tab and moved external forge accounts to the swarm
UI; credentials.html never had a matrix tab).
hivectl/README.md: agents.rs has no create verb (hivectl-cli.md has no
'create' entry; agents.rs's create is swarm-level, swarmctl agent
create). forge.rs reconciles config, it doesn't provision an account;
matrix.rs/github.rs do agent-scoped invites/token writes; gateway.rs
manages the gateway's own htpasswd users — split out of the former
single 'per-integration account/token provisioning' line.
An operator now links an agent's external forge account (label, base URL,
token) in the swarm UI. swarm-controller stores it at
swarm/agents/<agent>/forge/<label>. There is no index: the store's
listing of the agent's forge/ directory is the set of accounts.
In the agent, hive-agent-forge-accounts (oneshot + 2-minute timer, as
the agent user, under its own store certificate) lists
swarm/agents/<agent>/forge/ with the `list` #4866 grants an agent on its
own metadata subtree, reads each account, and writes
<state>/forge-<label>-token and forge-<label>.json in the names and shape
hive-forge -f already reads. An empty listing (a 404, which `bao kv list
-format=json` answers with `{}` and an empty stderr) is zero accounts; a
denial or an unreachable store fails the unit. It never deletes: files
for labels not listed, including ones the hive wrote, stay as they are.
Removed: the dashboard FORGES tab (credentials.js/html section and its
CSS), hive-c0re's extra_forges.rs and its routes, priv_client's
extra-forge calls, and hive-priv's WriteAgentExtraForgeAccount /
DeleteAgentExtraForgeAccount with their helpers. The GITHUB tab and
WriteAgentGithubToken stay.
Also: persistence.md's matrix avatar note names the exit-75 restart on a
changed account listing, not the dashboard, as what brings a linked
account up.

An ACP agent's model and effort pickers now list what its session offers
(its `model` and `thought_level` config options) instead of the claude
model list and EFFORT_LEVELS. A pick goes through the same Bus::set_model /
Bus::set_effort -> Config.model / Config.effort path as on claude; before
each prompt the ACP runtime sets it with `session/set_config_option`, model
first, and only when the session offers that value. Options are re-read
from the set response and from `config_option_update`, so the effort picker
disappears when the chosen model offers no effort levels, and is hidden
while a newly picked model waits for the next turn.
The session's options reach the web UI through a `Choices` handle from a
new `Runtime::choices`, registered on the bus the way `canceller` is.
/api/model and /api/effort accept only offered values on ACP. The claude
path is unchanged.

`/api/cancel` stopped a turn only by SIGINTing a child process whose argv0 is
`claude`, so on an ACP agent it reported "no claude process to interrupt"
and the turn ran on. The serve loop now stores the session's canceller on
the `Bus` when the runtime has one, and `/api/cancel` uses it: the agent is
sent `session/cancel`, and the next wake prompt carries the interrupted hint,
as for a signalled claude turn. Without a canceller (claude), the SIGINT path
is untouched.
An ACP turn stopped by the idle watchdog (`HIVE_TURN_IDLE_SECS`) becomes
`TurnError::AgentStall(note)`, handled like `ApiStall`: park for
`HIVE_STALL_SLEEP_SECS`, requeue, and record `api_stall`. The TurnEnd note
is the runtime's message (how long the agent was silent, whether it had to
be killed, and that a silently retried provider error such as HTTP 429
looks like this), not the claude-specific `ApiStall` text. A cancel the
agent ignores is a `Failed` turn.

swarm-controller's POST /api/agents now refuses (409) a name the swarm
has already placed on a different hive: a non-Destroyed declaration in
that hive's wanted state, or a SetAgentWanted node still queued for it.
The same name on the same hive is that agent being re-created and goes
through. A wanted state that cannot be read refuses (503/500) instead of
reading as "placed nowhere". Creations are serialised from that read to
the graph insert so two concurrent creations of one name cannot both
pass.
Hive-level creation is removed: hivectl `agent create` / `request-create`,
HostRequest::Spawn / RequestSpawn, the dashboard POST /api/request-spawn
route, and ApprovalKind::Spawn with its approve/resolve arms and the
approval-carrying `templates::spawn`. The swarm path (deploy request or
wanted-state sweep -> queue_first_deploy -> templates::first_deploy) used
none of them. Old `spawn` approval rows are skipped by collect_lenient,
as `init_config` rows were in a3b672d1.
policy.rs's comment on agent_object_name stated swarm-wide name
uniqueness as a fact; it now says where it is enforced and what that
check cannot see.

The CR3D3NTIALS page's MATRIX tab was the only caller of
`POST /api/matrix-account-login` (provision/log in an external matrix
account through the hive) and `GET /api/matrix-accounts` (its account
list). External matrix accounts are linked from the swarm UI now
(`LinkMatrixAccountForm` -> swarm-controller), so the hive-side UI and
both routes go. `priv_client::restart_matrix_daemon` had no other caller
and goes with them.
Already-provisioned credentials keep working: the `matrix-token-<name>`
files and `matrix-account-<name>.json` sidecars the old route wrote are
still discovered by hive-matrix-mcp (`accounts::configured` ->
`discover_token_accounts`), the `matrix-token*` path unit still re-fires
the daemon, and `WriteAgentMatrixToken` stays for the swarm credential
worker. Removing that usage waits on moving the existing creds to
swarm level.
The GITHUB tab is the credentials page's default tab now.

Per mara's review on #4769: don't extend stale info to say it's stale,
just remove it. The sentence only described the absence of the removed
topology tree, giving the operator nothing actionable.
C0NTAINERS already states the flat/alphabetical/no-nesting fact;
the section repeated it with zero new operator info. Fix the two
dangling references (README.md reading-path index, swarm.js
comment) rather than leaving them pointing at a removed heading.
The backend dropped the agent hierarchy's parent field, so every
container is a root and buildAgentTree/treePrefixDom could only ever
produce a single-level flat list — the .tree-prefix CSS lane rules
already matched nothing. Replaced with sortedContainerRows, a plain
alphabetical sort, and dropped the now-dead .tree-prefix/.tree-lane/
data-depth CSS and the depth/isLast/ancestorIsLast fields from the row
fingerprint and buildContainerLi's signature. No visible behavior
change - the rendered list was already flat, just via dead machinery.
Docs updated to describe the simpler implementation directly instead
of narrating the removal (kept the heading name since swarm.js still
points a comment at it).
mara's ruling on the review: documentation describes functionality as
is. The dashboard doc carried a "Topology tree" section marked dormant,
still spelling out the indent lanes, joints and continuation bars the
renderer paints — for a renderer that, with no parent field to walk,
puts every container at depth 0 and emits no prefix column at all. A
section labelled dormant is still a section describing a feature the
code does not have.
Each one now states what the page renders today: SW4RM's C0NTAINERS is
a flat alphabetical list, one row per container, no indent and no glyph;
the tree section says nothing nests and names the code that decides so;
the selection bar lists the bulk actions it has, without a note about
the M0V3 picker it doesn't (agent-hierarchy.md's removal table is where
that record belongs).
Two more the -U15 context sweep turned up outside that section, neither
naming a removed identifier so neither reachable by grep: approvals.md
told an agent to clone "the child's" config repo, and hivectl.md sold
`agent restart` as a way round "the agent hierarchy".
The prose added by this branch named the tracker item in seventeen
places, which check-issue-refs.sh rejects: a `#N` tag is dead weight for
anyone reading the public mirror, where no issue data exists. Each one
now states the fact it was pointing at — the parent field is gone — so
the sentence stands on its own.
Two of those lines also carried a rustdoc break: `[`write`]` in
topology.rs is ambiguous between the module's own `write` fn and the
`write!` macro, which `-D rustdoc::broken-intra-doc-links` fails. Spelled
`[`write()`]`, per rustdoc's own suggestion.
The host_config.rs rewrite is two lines rather than three so the doc
block stays under check-comment-blocks.sh's 30-line ceiling.
The topology doc keeps its filename and its second half (manager
special-casing, harness unit shape) — both are cross-referenced from
other pages and neither is about the parent field. Its first half is
rewritten: what topology.json is now, and a table of what the removal
took with it, so a reader who finds `<parent>` or `set-parent` in an old
issue thread learns it went away rather than moved.
The dashboard's tree-rendering section is marked dormant rather than
deleted: the walk is still in swarm.js and retiring it is the frontend
owner's call.
swarm-logs covers every host-tier unit this tool could reach, so the
second, capability-gated path into host journald earns nothing and is
removed outright rather than disabled behind a flag.
Removed end to end: the MCP tool definition + handler, the
GetHostJournal/HostJournal wire variants, hive-c0re's
dispatch_host_journal handler, the ReadHostJournal capability, and the
harness-side capability->--allowedTools gate. get_host_journal was the
only capability that mapped to an MCP tool, so allowed_capability_tools
could only ever return an empty vec; it goes too rather than linger as a
function that provably does nothing.
capabilities::has_cap/caps_for stay: #4624 gave ManageRootAgent's
bind-mount enforcement (hive-c0re/src/lifecycle/host_config.rs) a
second caller of has_cap, so they're no longer callerless once this
lands on top of it.
hive-sh4re's journal module (JournalPriority) had no consumer outside
this tool and is deleted.
An existing capabilities.json still naming read_host_journal does not
error: capabilities::prune_unknown drops unrecognised names with a
warn!, and an agent left with no capabilities has its entry removed. No
migration step is needed.
Untouched: hive-c0re/src/dashboard/journal.rs's
read_host_journal_response, which matches the name but is the private
helper behind the operator-only GET /api/journal-host dashboard route
and carries no capability check.
The hive had two spellings of "this agent may act on agents that aren't
its children": the `ManageRootAgent` capability, which nothing checked,
and a `can_manage_top_level_agents` role in a third meta store,
`roles.json`, which owned the real grant — the bind mounts that put
another agent's state (rw) and config (ro) inside the holder's
container. The two drifted independently, and with the parent/child
hierarchy removed the role's set (`parent.is_none()`) silently became
every agent while nothing said so.
Collapse them. The mount grant now hangs off
`Capability::ManageRootAgent`, looked up through the one capability
path that already exists (`capabilities::has_cap` over
`capabilities.json`) rather than a second mechanism. `roles.json` and
everything that read, wrote or reconciled it is gone, along with its
`meta.rs` staging and commit-label wiring; nothing in the tree reads
that file any more.
The enum variant keeps its name deliberately. Renaming it would turn
every `manage_root_agent` already stored in `capabilities.json` into an
unrecognised name that `prune_unknown` drops without asking. Its
meaning, not its spelling, is what changed: "may manage any agent". The
doc comment and the description string now say that.
`top_level_agents()`/`top_level_agents_in()` are replaced by
`all_agents()`/`all_agents_in()`. Under "manage any agent" the mounted
set is every agent by definition, so the code states it instead of
deriving it from a predicate that no longer discriminates — and the
call-site comment explains that, because it otherwise reads as a
widening. The holder is no longer bound as its own virtual child: that
reproduced the own-state and own-config mounts exactly, so dropping it
loses nothing.
133 hits across 38 files, all previously classified during #4548's sweep
and deliberately left un-rewritten (predicate-adjective state/necessity
description, design-intent idiom, structural/type-description idiom,
no-single-actor topology claim, parallel-triple exception, vale
substring-match artifact — see hyperhive#4548's per-PR bodies for the
per-hit reasoning).
Wraps each one in a scoped <!-- vale write-good.Passive = NO/YES -->
pair (the supported mechanism — TokenIgnores has a known offset-drift
bug) rather than a blanket per-file or per-rule silence, so a *new*
passive-voice hit anywhere in these files still fails once the rule
gates CI (next commit). Table/list false positives (docs/swarm/credentials.md's
renewal-table cells) wrap the whole block, not each cell.
Part of #4546.
5 genuine passive-voice rewrites (actor named — DOMPurify.sanitize()
x2, paintAtomic, the keyed pattern, the operator); 2 false positives
left untouched — predicate-adjective state descriptions ('are
unaffected', 'is gone'), same category as several already-established
in this sweep.
get_loose_ends now always returns the caller's own loose ends, for every
caller including the manager (ruth) — there is no separate manager
surface, ruth is a normal agent with different default capabilities.
- AgentGetLooseEndsArgs removed; get_loose_ends takes no args.
- Wire Request::GetLooseEnds collapses from an Option<String> target to
a unit variant.
- hive-c0re's handle_get_loose_ends drops the "*" hive-wide branch and
the subtree/capability resolver (resolve_agent_state_target); both
are gone since there is no longer a target to resolve.
- loose_ends::hive_wide and Capability::QueryAgentState removed as
dead code — their only callers were the two functions above.
- is_descendant_of is untouched (still used by lifecycle_handlers.rs
and schedules.rs independently of this change).
- Docs updated: docs/turn-loop/mcp.md, docs/web-ui/dashboard.md,
docs/process/conventions.md (Loose-ends wire shape + capabilities
table), plus the doc comments in hive-core-agent-sock, mcp_config.rs
and capabilities.rs that described the old shape.

Part of #4548 — rewriting the 353 hand-written-docs write-good.Passive
hits so #4546 can flip the rule to error. This batch: docs/networking/gateway.md
(27 hits), docs/tools/subagent.md (19), docs/web-ui/dashboard.md (18).
Minimal, meaning-preserving rewrites to active voice only — no
restructuring beyond what each flagged sentence needed. Genuine false
positives (predicate-adjective state descriptions like "is required",
"is broken", "is unchanged", adjectival past participles like "named
buckets", and one quoted literal UI string) are left in place; the
per-line reasoning is on the tracking issue.
Note: docs/tools/forge-cli.md (37 hits, would have been the single
biggest file) turned out to be generated from hive-forge's own clap
help text (see hive-forge/src/main.rs's `MarkdownDocs` verb) — same
category as the already-excluded hivectl-cli.md/swarmctl-cli.md, so
it's untouched here. Flagged on #4548.
Container lifecycle from inside an agent goes away: an agent no longer
starts, stops, restarts or rebuilds a container in its subtree, and no
longer reads another container's journal. Those are operator actions —
the dashboard and hivectl keep their own paths to the same job-queue
and hive-priv plumbing, which is why none of that machinery is removed
here, only the five MCP verbs and what they alone reached.
What went with them: the `Request` variants and `Response::Logs` on the
agent socket, the five tool definitions and their arg structs, the four
lifecycle handlers plus `handle_get_logs`, and `require_descendant` —
the topology guard those five were the only remaining callers of.
`ToolGroup::Diagnostics` goes too: `get_logs` was its only tool, so it
would otherwise be a grantable group that grants nothing. `lifecycle`
stays, now carrying `list_containers` alone.
An agent that gets a `needs_update` or `container_crash` helper event
has no remedy of its own left, so the system prompt and the docs now
send it to the operator instead of to a tool that no longer exists.

swarm-controller's `InitAgentConfigRepo` node already covers config-repo
creation, so this deletes a duplicate rather than a capability; old
`init_config` rows are skipped by `collect_lenient` with no migration, by
operator decision.

Moves the TermMsg rendering pipeline (Row.tsx, termMsg.ts, linkify.tsx,
markdown.ts) from @hive/agent into @hive/shared, so swarm-ui becomes a
second consumer of it instead of forking a copy -- CSS was already
shared (@hive/shared/terminal.css). marked+dompurify move from
@hive/agent's deps to @hive/shared's; swarm-ui picks them up
transitively, no new direct dep there.
New swarm-ui route /agents/:name/term (AgentTermPage), linked from
AgentsPage's detail panel via a "terminal" badge next to "link matrix
account". Consumes GET /api/agents/{name}/term/stream: unlike
@hive/agent's own useLiveStream (TermEnvelope-wrapped, history/backfill
dance), the swarm relay forwards one bare TermMsg per SSE event with no
envelope and no history endpoint -- useSwarmTermStream is a much
smaller hook for that shape (EventSource -> parse -> coalesce, nothing
to buffer/dedupe/backfill against).
Verified against a live SSE mock (screenshots in /agents/iris/state/screenshots/
3801-agents-detail-panel-terminal-badge.png and
3801-agent-term-page-live-rows.png -- real rows rendering through the
shared Row component, not just a build/typecheck pass).
A terminal row published on `$SWARM.term.<hive>.<agent>` goes out bare,
with no envelope around it and no server-side stamp, so a subscriber had
nothing to place the row in time with beyond its own receipt clock —
wrong by the queue's latency and meaningless for anything read later
than live.
`TermMsg` gains `ts`, ISO 8601 UTC. `classify` takes the event's own
unix-seconds stamp and applies it to every row that event expands into,
so a row replayed out of sqlite says when it happened rather than when
it was read, and a row that sat in a lagging subscriber's buffer does
not lie about its time. The oversize degrade keeps it; only the body is
ever spent.
`TermEnvelope` stops duplicating `ts` and keeps `seq`: the dedup counter
is a real transport concern, the event's time is not, now that it rides
on the row. Nothing in the frontend read `envelope.ts` — only the type
declared it.

Per mara's screenshot report (agent-terminal icons not aligned in the
first column) — and her explicit follow-up steer on the first version of
this fix: 'dont do it by offsets at all, it should be part of the layout
that they align.'
Root cause traced first, not guessed: .row-glyph relied on inheriting
text-indent: -1.4em from .live .row to paint its glyph in the reserved
prefix slot; details.row > summary separately reset text-indent: 0 for
its own hanging-caret needs, which also zeroed the inherited value for
the icon nested inside it (indent inherits by computed value, not by
rule). Two independently-computed offsets that had to agree by hand,
and silently didn't.
Replaced the whole offset scheme with a real structural fix: every row
is a 2-column CSS grid (icon column, width from one shared
--row-icon-col custom property, then content column), and a details
row grids its own <summary> with the exact same grid-template-columns
value instead of griding itself (so its <pre> body still stacks full
width below, not squeezed into column 2). Icon and content are placed
by explicit grid-column, not auto-placement inference, so an icon-less
row's lone child still lands in the content column. .row-content is a
new wrapper class (Row.tsx, terminal.js's row()/mutableRow()/
placeholder()) giving that content an element the grid can target by
class - a DocumentFragment (what linkify() returns) doesn't persist as
a node once appended, so without an explicit wrapper there was nothing
for the grid to place.
Caught and fixed a second real bug while building this for real (not
just reasoning about the CSS): the first draft's details.row { display:
block } had lower specificity than .live .row's display: grid and never
actually applied, squeezing <summary> into the row's own 1.4em icon
column and wrapping its text one character per line. Needed
.live details.row to out-specify it.
Verified with a headless-chromium render of all 6 row shapes (flat
icon / flat icon-less / flat markdown-body / details icon / details
icon-less / a long-wrapping flat row) - all align and wrap correctly.
Also verified via the earlier Range.getBoundingClientRect() measurement
(glyph paint position, not just the element's own box): diff 0.00 for
both this and the previous fix, but only this one is structural rather
than two numbers that happen to still agree today.
Per #4128 (mara: allow-everywhere false positives go in a central
list, otherwise fix in source). Testing surfaced better fixes than
the plan posted on the issue:
- 5x Microsoft.Contractions 'that is' idiom false positives: adding
the missing comma ("that is, ...") both reads better and satisfies
the rule's own negative-lookahead, so no suppression is needed at
all. Fixed in docs/integrations/forge.md, docs/tools/forge.md,
docs/tools/hivectl.md, docs/web-ui/dashboard.md, and
swarmctl-cli.md's generated source (swarmctl/src/main.rs, doc
comment regenerated via markdown-docs).
- persistence.md's 'is not' matching inside 'is nothing': reworded to
'there'\''s nothing' rather than add any exception -- dodges the trap
and is a genuine contraction besides.
- ca.md's 'it is' matching inside the already-correct 'it isn'\''t':
tried a central .vale.ini TokenIgnores entry first per the
allow-everywhere framing, but testing against the real file (not
just a synthetic snippet) found it silently fails to suppress
whenever markdown emphasis syntax appears earlier in the same
file -- an offset-drift bug in how Vale applies TokenIgnores, not
a config mistake. Reworded to "it'\''s not" instead, same fix
shape as persistence.md.

provider, matching the nix option's own name, not one internal
system to name): scoped inline vale suppression around just that
section, since this one really is context-specific rather than a
rule bug.
Verified: fresh 'vale docs/ --minAlertLevel=error' is 0 errors AND
0 warnings (was 10 errors). nix fmt 0 changed beyond the edits
themselves. pre-push lints (tracker-tag/comment-block/doc-pointer)
clean. cargo clippy -p swarmctl -- -D warnings clean. Diffed the
regenerated swarmctl-cli.md against the old copy to confirm only
the intended line moved.
Per mara's ruling on hyperhive#4041 (Microsoft.We): keep the rule enabled,
same treatment as the 'backend' rewrites. Traced all 21 genuine hits (4
gateway.md 'Let's Encrypt' hits are a substring-match false positive, left
alone) to their actual referent: some name a specific component already
established nearby in the same doc (forge_notify, hive-github-notify's
poller, hive-forge, hive-agent/the harness, colors.css, the dashboard),
others were pure filler that adds nothing once dropped.
