cafa120d77 swarm-builder: scope the forge token to the forge, one build at a time, opt-in
argus's review on #5177: the git credential helper answered every `get`,
so a config PR's own flake inputs could have git send the builder's forge
token to any host. The helper is now configured under
`credential.<forge-url>.helper` and itself answers only an https request
whose host is the forge's; a test runs it as git does and checks that
other hosts, http and non-get operations get nothing.

Builds are bounded: every `build_agent` node needs the one `builder` jobq
resource, so one runs at a time and the rest wait pending, and a newer
head cancels its PR's pending build (a running one finishes and writes on
its own commit). The builder also holds a one-permit semaphore around each
build.

`builder` and `review-classifier`, the forge logins swarm-controller owns,
join nix/reserved-names.nix, with a test tying the list to both constants.

The builder is now opt-in like the CI runner: `allSwarmServices` no longer
turns it on, since it runs unreviewed config PRs through the host's nix
daemon. mara had not answered atlas's question on the PR.

7e6a4c7727 agents: remove send(operator) and the operator inbox; alert on turn failures
Agents reach the operator over matrix now (#5115). A `send` to
`operator` is refused, by the broker and by the MCP allow-list check,
with an error that names the matrix MCP tools. The dashboard's Y3R C4LL
tab goes with it: the operator inbox, `GET /api/operator-inbox`,
`Broker::unread_for_recipient`, and the browser notifications that only
ever fired on operator-bound messages.

The harness no longer posts turn failures and plugin-install failures
to the operator inbox. Both are already logged at warn; a new Grafana
rule, `hh-agent-turn-failed`, fires per agent on the harness's
`turn failed` line.

Dashboard-to-agent messaging (op-send, the agent web UI's message box,
mark-all-read for agent inboxes) is unchanged.

Decisions per mara on #5115: q1 a, q2 a plus a grafana alert, q3 keep
dashboard-to-agent messaging and leave swarm-level code alone.

1c90619fbb nix: comments state current behaviour, not history
Rewrite comments and option descriptions under nix/ that narrate how the
code got here ("used to", "new name", "sat at the TOP", "no longer",
renamed/moved narration, attributions) to say what the code does now and
why. Comments and description strings only; no logic changes.

Files in open PR #4970's diff are left alone. Refs #3902

58ef299c3c hive-c0re: remove the approvals subsystem
Nothing submits an approval since scheduled prompts left hive-c0re, so the
queue only acted on rows written before that.

- hive-c0re: the approvals store and its migrations, approve/deny/
resolve actions, the ResolveApproval tail node (and meta_update's
approval_id), POST /api/approve and /api/deny, the approval views on
/api/state, the approval_resolved dashboard event, the orphan GC,
destroy/purge failing pending rows, and the approval rows in
get_loose_ends. DestroyBookkeeping loses the purge flag, which only
  worded the approval-failure note.
- The CancelLooseEnd request goes: its only hive-c0re arm was the
approval one. cancel_loose_end handles reminder and todo in the harness.
- Wire types: hive_sh4re::approvals (ReminderStats moves to inbox),
  LooseEnd::Approval, CancelLooseEndKind, HelperEvent::ApprovalResolved,
  ToolGroup::Approvals, HostRequest::{Pending,Approve,Deny} and
HostResponse.approvals. HOST_SOCK_VERSION goes to 2 for the removed
  field.
- hivectl loses the approvals verbs; docs/tools/hivectl-cli.md is
  regenerated.
- The system prompt drops approval_resolved.
- The approvals table stays in broker.sqlite, unread, the same way the
operator_questions table was left when its store was removed.
- An `approvals` entry still in tool-groups.json is skipped with a warning
by ToolGroup::parse_list, like any unknown group.
- docs/agent-lifecycle/approvals.md is renamed to config-changes.md so the
code's doc pointers resolve; its content follows in the docs commit.

88e571a463 swarm-controller: refuse a new agent name the forge would reject
Every agent becomes a Forgejo user of the same name, and nothing upstream
of `CreateForgeUser` knew what Forgejo refuses: `admin`, `api`, `foo-` or a
41-character name passed name validation and failed one node into
provisioning with Forgejo's 422.

`nix/reserved-names.nix` gains the 22 reserved usernames of Forgejo
v16.0.5 (`models/user/user.go:639-680`) that `[a-z0-9-]` can spell, and
the bare `-` (`models/repo/repo.go:67`). The dot and underscore entries are
left out, since our charset cannot produce them. The header's admission
rule grows a third class — a username the forge refuses — because that is
a failure behind the refusal.

The shape rules are not literals, so they live in
`hive_types::forge_username_violation`: no leading `-`, no `--`, no
trailing `-`, at most 40 characters. Beside `is_reserved_name`, not in
`Ident::parse`: an `Ident` is also a hive, label, account and subagent
name, and parsing runs on every read of an existing name.

`create_agent` used to WARN on a reserved name, deliberately: an operator
with agents already created under a colliding name would otherwise be
unable to re-run creation. That reason is kept, and narrowed to what it
protects. A name breaking either rule is now refused with a 400 naming the
rule when the name is NOT in the swarm roster, and still only warned
about when it is, so re-creating an existing agent keeps working. The
roster is read only for a rule-breaking name; when it cannot be read, a new
name and an existing one look alike, and this warns as before. The
hive-collision warning is unchanged.

179f873722 docs: retire the agent hierarchy from every page that described it
The topology doc keeps its filename and its second half (manager
special-casing, harness unit shape) — both are cross-referenced from
other pages and neither is about the parent field. Its first half is
rewritten: what topology.json is now, and a table of what the removal
took with it, so a reader who finds `<parent>` or `set-parent` in an old
issue thread learns it went away rather than moved.

The dashboard's tree-rendering section is marked dormant rather than
deleted: the walk is still in swarm.js and retiring it is the frontend
owner's call.

a3b672d1d5 refactor(hive-c0re): drop the request_init_config tool and InitConfig approval
swarm-controller's `InitAgentConfigRepo` node already covers config-repo
creation, so this deletes a duplicate rather than a capability; old
`init_config` rows are skipped by `collect_lenient` with no migration, by
operator decision.

cdaf5824ed swarm: refuse hive names that CONTAIN a word identifiers are composed from
Hive-scoped identifiers are built by joining a hive name with fixed words —
`hive-<name>`, `hive-<name>-agent` — so a hive called `foo-agent` produces
exactly the id hive `foo`'s agent containers produce. The queue's responder
resolves it as the agents, and that hive silently receives an agent grant
instead of its own; a NATS denial reaches a client as a timeout, so nothing
names the cause. Equality cannot see this: the two NAMES differ, only the
composed strings collide.

Deliberately a second file rather than three more entries in
`reserved-names.nix`. That list is matched by equality against a whole name and
every entry is a value some component emits; applying containment to it would
refuse `forgeworks` and `operator-hq` for failures that do not exist. The two
lists differ in subject (hive-only vs both), matcher (substring vs equality)
and admission rule, and each file's header says so.

No Rust change: `HIVE_RESERVED_NAMES` answers "may an AGENT be called this",
which stays an equality question — an agent name is never composed into these
identifiers.

Measured against the predicate rather than assumed:

  pr1ma=ok alpha=ok forgeworks=ok operator-hq=ok
  foo-agent=REFUSED beehive=REFUSED swarm=REFUSED myswarmx=REFUSED

The four `ok` cases are the control: `forgeworks` and `operator-hq` are exactly
what a merged list would have refused.

27932ec631 types: let nix own the reserved-name blacklist
One list, in nix/reserved-names.nix, handed to everything that needs it
as HIVE_RESERVED_NAMES. Keeping it current becomes a config change
rather than a rebuild, and hive names and agent names -- one namespace
going forward -- are checked against the same file: swarm-otel.nix's
hand-written reservedOwners is gone.

Whitespace-separated rather than JSON, deliberately, unlike the
structured env vars beside it. Every entry is an Ident ([a-z0-9-]), so
whitespace cannot occur inside a name and the encoding is provably
lossless; JSON would mean either a parser dependency in a crate whose
purpose is to have none, or a copy of the parse in every consumer.

An UNSET variable is not "nothing is reserved". Both creation sites log
an error and return a warning saying the check did not run, so a
misconfigured deployment says so instead of silently accepting every
name. A blank value folds into unset: nix always renders a non-empty
list, so present-but-empty is a rendering fault, not a declaration.

Two guards whose subject moved out of their own file now assert their
own case is still in it, because a guard that can be retired by an edit
elsewhere is not a guard:

- swarm-otel.nix asserts reserved-names.nix still contains its
  swarmTierName.
- hive-sh4re's sentinel drift test PANICS when the variable is missing
rather than skipping -- a drift test that quietly does nothing still
reports green. checks.nix and devshell.nix both export it so CI and a
local cargo test agree. Verified as a pair: with the variable set, 8
tests pass; with it unset, exactly the 4 drift tests fail and the
  unrelated ones still pass.
